1016476901 2018-09-11 Production Wells No Violations The initial compliance notice issued on 8-10-2018 while conducting an order follow-up for Chiefs Order 2016-391 found in Inspection # 1533921014 is passed as of 09-11-2018. This is due to an error which the date to remedy the defective casing conflicting with the 180 days to plug. A new inspection was done and a new compliance notice was issued citing 12A in 1509 of the Ohio Revised Code.
1029320461 2018-07-12 Field Inspected, Well Not Found No Violations I did an Order follow-up inspection (Chiefs Order 2016-391) for this well. I did not locate the well during my inspection. The well is located between the between the Ohio Canal and the western edge of the Tuscarawas River. The tank battery consists of a steel production tank, a plastic brine tank and a vertical separator. The well identification on the production tank can be read but is faded. The record shows the well last produced in 2012 for 1080 MCF of gas. Access to the well can be gained via the towpath trail. The tank battery access is at these coordinates +/- 9 (41.01296, -81.60173). I will follow-up with another inspection to locate the well.

1533921014 2018-08-10 Production Wells Violations Noted With Bob Roush (MRI-3), I did an Order follow-up inspection (Chiefs Order 2016-391: Failure to Maintain Liability Insurance) for this well. The well is equipped with a plunger-lift system and is shut in. The well is located between the between the Ohio Erie Canal Towpath and the western edge of the Tuscarawas River. The tank battery consists of a steel production tank, a plastic brine tank and a vertical separator. The well is legibly identified. The tank battery access is at these coordinates +/- 9 (41.01296, -81.60173). Access to the well can be gained at these coordinates +/-9 (40.01590, -81.60200) on the towpath. While moving towards the well, escaping gas could be heard leaking approximately 50 ft. away. The 1.5 inch tubing is leaking gas by two band clamps located between the gate valve and tubing head. The last reported production for this well was in 2012 for 1080 MCF of gas. The owner shall immediately repair the casing, correct the construction inadequacies and produce or plug and abandon the well. Consequently, this well shall be plugged in accordance with Ohio Revised Code section 1509.12(B). The well owner/operator shall submit a written plugging schedule to Shawn Murphy at 3575 Forest Lake Dr., Ste. 150, Uniontown, Ohio 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

A phone call was made to Mr. Mike Schreiner at 330-323-2277 and a message was left requesting a return call to Shawn Murphy (MRI-2) or the Uniontown Office of Oil & Gas.

1535647558 2018-08-30 Production Wells No Violations I was on location with Inspector Bob Roush to follow-up Chiefs Order #2016-391 for failure to maintain liability insurance and Compliance Notice #1533921014 issued for defective casing and requiring that the well be plugged. My inspection today found that the well continues to leak natural gas to the atmosphere above the tubing head and has not been repaired as requested. The escaping gas is audible at ~ 50 ft. from the wellhead and the location and current state of the leak poses a risk to public health and safety. I left a message with the well owner, Mr. Mike Schreiner (330-323-2277), requesting a return phone call to myself or to the Uniontown Field Office to address the current safety issue with this well. Further enforcement action is being pursued against Schreiner Oil & Gas Incorporated to resolve this issue.
