0268433721 2015-05-27 Plug / Plug Back No Violations I was on the Semansky Unit # 1 location to witness the P&A activities. Bishop Well Services ( BWS ) ran in the 1.5 tubing and tagged the cement top at 3291. Appalachian Wireline Surveys ( AWS ) shot the tubing at 2596 and was pushed out of the tubing when the well equalized the annular and inside fluids. BWS removed 79 joints of 1.5 tubing from the 4.5 casing. AWS set a wirline bridge plug inside the 4.5 casing at 2550. The required pressure testing of the bridge plug failed as it would not hold the pressure enough to meet the test criteria. Woody McDaniel ( Enervest ) decided to set a second bridge plug on 5/28/2015. No violations identified during this inspection.
0282060520 2015-05-26 Plug / Plug Back No Violations I was on the Semansky # 1 location to witness the placement of the Clinton sand plug. Bishop Well Services ( BWS ) removed the broach from the 1.5 tubing and went back in the hole with slim hole rods to remove the plunger from the tubing. After the plunger was recovered the 1.5 tubing started to flow a 9.8 ppg fluid at a rate of 4 gpm. Petroset Cementing Services ( PCS ) connected to the 1.5 tubing, mixed a cement slurry comprized of 60 sacks of Class A blended with 2 % gel and displaced it through the interval 3870 to 3070. The well ( casing and tubing ) was shut in after completion of the displacement. No violations identified during this inspection.
1155005178 2015-05-29 Plug / Plug Back No Violations I was on the Semansky Unit # 1 location to observe the plugging of the well. Bishop Well Services ( BWS ) tagged the cement top in the 4.5 casing at 2000 ( I was unable to witness the setting of a second bridge plug and the successful pressure testing conducted on 5/28/2015 ) and ripped the casing at 1807. BWS recovered 43 joints of 4.5 casing. BWS placed 2.375 tubing at 1778 to set a rip plug. Plugging operations were stopped due to scheduling with Petroset Cementing Services and Appalachian Well Services. No violation were identified during this inspection.

1425331501 2015-05-22 Plug / Plug Back No Violations I was on the Semansky Unit # 1 location to witness the P&A activities being conducted by Bishop Well Services ( BWS ) for Enervest Operating. BWS ran a tapered broach in the 1.5 tubing on 108 slim hole rods and tagged the obstruction at 2712. BWS continued to broach the 1.5 tubing until the obstruction was removed. No violations were identified during this inspection.
1432129039 2015-05-19 Plug / Plug Back No Violations I was on the Semansky Unit # 1 location to conduct the required pre-plugging assessment of the wells condition due to its proximity in an area known to flow fluids out of the Salina Formation. The well is in a location where PPG conducted solution mining activities since the late 1890s. During the drilling of the Semansky Unit # 1 it was reported that flows were encountered. Bishop Well Services ( BWS ) is the plugging contrator for Enervest Operating. The well is constructed with 8.625 ( 340 cemented to surface, 3992 of 4.5 casing cemented with 125 sacks of cement ( 3992 to 3192 approx. ) and Two Stage cementing tool set at 2592 cemented with 100 sacks ( 2592 to 1892 approx. ). Tubing was ran in the 4.5 casing to approximately 3700. Upon examination the 8.625 annulus registered 0 psi, blew down and stopped. The 4.5 casing had 20 psi and flowed to the 140 barrel stock at an estimated rate of 40 barrels per hour. The fluid parameters, sampled from the 4.5 casing, were 9.1 ppg, 6 - pH, 2019mg/ L - Cl and a blue tint. The 1.5 tubing flowed a small amount of oil and then loaded up. The 4.5 casing fluid turned clear and then weight increased to 10.1 ppg. The well was shut in and secured pending arrival of equipment on 5/20/2015. No violations noted during this inspection.
1433508698 2015-05-21 Plug / Plug Back No Violations I was on the Semansky Unit # 1 location to witness the P&A activities on the well. Petroset Cementing Services ( PCS ) was on location to pump on the 1.5 tubing and try and establish a flow rate to allow placement of a cement slurry across the Clinton sand perforations. PCS connected to the 1.5 tubing and after pumping 2.5 barrels of fresh water the pressure rose to 1600 psi. PCS was released from location. Bishop Well Services ran in the 1.5 tubing with a stem and jars and reached a depth of 3870. Since the 1.5 tubing was hung in the 4.5 casing, Appalachian Wireline Surveys ( AWS ) were employed to shoot off the 1.5 tubing. AWS ran into the 1.5 tubing with a tubing cutter, but could not get deeper than 2707. AWS was released from the location. Decision made made by Woody McDaniel ( Enervest ) to broach the tubing to allow AWS to get the cutters deeper. No violations were seen during this inspection.
1433867961 2015-05-19 Plug / Plug Back No Violations I was on the Semansky Unit # 1 to conduct a pre-plugging evaluation of the annular spaces in the well due to its proximity to the PPG Solution Mining operations. Bishop Well Services ( BWS ) was the plugging contractor for Enervest Operating LLC. BWS personnel exposed the valves on the 8.625 casing head. The annular space ( 8.625 X 4.5 ) had a light flow of gas and blew down. The pressure was too light to gauge and no flow was observed. The 4.5 X 1.5 annular space pressure was measured at 20 psi and was flowing a blue brine ( 2019 mg/L of Cl & 9.1 ppg ). Flow rate was measured at ~ 45 barrels per hour. It was determined that the 4.5 casing integrity was breached since the casing had been cemented with a multi - stage cement job above the Salina section and the absence of flow out of the 8.625 annular space. The 1.5 tubing was measured to have 120 psi, but would not flow to the tank battery. The fluid flowing out of the 4.5 casing changed characteristics. The fluid turned a black color with chlorides being 620 mg/L ( Quantabs ) after one (1) hour of flowing. The well was shut in and secured for the night. Plugging operations to commence on 5/20/2015. No violations were identified during this evaluation.
1582927196 2015-05-20 Plug / Plug Back No Violations I was on the Semansky Unit # 1 to observe the P&A activites. Bishop Well Services (BWS ) crew unpacked the tubing head and tried to pull on the 1.5 tubing. BWS pulled to 20m over the string weight and did not move, thus indicating the tubing was hung in the 4.5 casing. BWS ran 1.5 tools ( stem and jars ) to see if the tubing was clear deep enough to allow shooting off the tubing and proceed with setting the bottom plug. The tools set down at 2750 GL per the depthometer. A solvent truck ( Flo-Rite ) pumped down the 1.5 tubing and pressured upto 300 psi. After several pressuring and releasing cycles the pressure fell back to 120 psi after 252 gallons had been pumped. The solvent truck could not establish a flow rate high enough to determine ( with celloflake ) if the tubing was crimped or fluid was going out the mud anchor. Decision was made to have Petroset Cementing Services pumped on the well in the morning. Shut down overnight. No violations identified during my inspection.
2114733583 2015-06-01 Plug / Plug Back No Violations I was on the Semansky location to observe the setting of a rip plug , bottom of the surface casing plug, bridge plug and surface plug. Petroset Cementing Services ( PCS ) connected to the 2.375 tubing and mixed, displaced a plug ( 13.9 ppg ) through the interval 1778 to 1578. Bishop Well Services (BWS ) removed 42 joints of 2.375 to 462. PCS mixed and circulated a cement slurry ( 14. 4 ppg ) through the interval 462 to 262. BWS removed the remaining 2.375 tubing and Appalachian Well Surveys ( AWS ) set a bridge plug at 230. BWS ran seven (7) joints of tubing to 214. PCS established circulation and circulated a cement slurry ( 14.3 ppg ) back to surface. BWS removed the seven (7) joints of 2.375 tubing and PCS topped off the hole to grade. After 30 minutes I observed the 8.625 casing was static and the cement level was at grade. No violations observed.
