0135120743 2017-10-06 UrbanProduction Wells No Violations I conducted a follow up inspection to assess the level of compliance with the actions required in Compliance Notice # 1601737730. The Compliance Notice detailed the following remedies : 1.) remove leaking 100 barrel brine tank, 2.) remove outdated I D signage, 3.) remove unused line pipe from location. My inspection determined that compliance was not met. The Compliance Notice # 1601737730 remains in effect.
1400895438 2018-02-12 UrbanProduction Wells Violations Noted I re-inspected the Lubanovich # 1 to assess the level of compliance with OAC 1501:9-9-05. A previous inspection ( # 1601737730 ) detailed violations including, a leaking 100 barrel brine tank and I D sign age which did not reflect the current operator. Additionally, there was non-essential piping and fittings at the tank battery. I found the 100 barrel brine tank and the out of date I D signage removed. The I D signage reflects the current Operator. The piping and fittings removed from the tank battery. The requirements in Compliance Notice ( # 1601737730 ) had been satisfied.
1601737730 2017-09-21 UrbanProduction Wells Violations Noted The Lubanovich # 1 was accessed on a gravel road. The well was operated as a 1.5 tubing plunger lift well. The wellhead was fenced and locked. The I. D. signage reflects the previous Operator. The current Operator shall remove the old signage. The tank battery consisted of one (1) 100 barrel stock tank, one (1) 100 barrel brine tank and one (1) horizontal separator. The tank pad area was surrounded with an earthen dike. The load lines were plugged. The required I. D. was posted and was current. The gas meter reflected the current production period. I found that the 100 barrel brine tanks integrity had been compromised. The tank was leaking at the bottom weld seam six (6) inches to the right of the two (2) inch drain valve. The gravel below the leak was freshly dampened. In order that compliance with OAC 1501:9-9-05 the Owner / Operator shall: 1.) remove the leaking 100 bbl. Brine tank and remediate any contamination underneath said tank, 2.) remove the outdated signage on the wellhead ( could be confusing in emergency ), 3.) remove yellow plastic coated line pipe from tank battery and leasehold. These compliance remedies shall be completed by 10/13/2017. Please contact Bob Roush at 330.690.8837 prior to remedial activities being performed.
