1331062486 2012-03-06 Production Wells Violations Noted During a meeting with Mr. Helms on 8/5/2011, he agreed to empty, remediate and remove the multitude of bottles, buckets and cans he uses to collect oil from inside of the secondary containment dike. He also agreed to post the required identification (including owner name, well name, location, permit number and emergency phone number(s)) on the fence or on a clearly visible storage tank. As of todays date, none of this work has been performed as agreed upon. This work must be completed within the next 30 days or additional corrective action will be taken. Furthermore, this well is for domestic use only. Any attempt to provide gas to the aparment building or to sell the natural gas through the Dominion East Ohio system will result in additional corrective action unless the required bond and liability insurance are obtained and documented.

1374700615 2004-11-08 Production Wells Violations Noted The inspection was done at the request of the OEPA which is in the process of preparing a case against the landowner for an unpermitted sewage treatment plant. The only MRM rules violation is no identification on either the tank battery or the wellhead. I will contact the landowner and request that the required information be posted.

1939720685 2011-08-05 Production Wells No Violations Barb Holdren, Green Zoning Inspector, requested this inspection of the well and especially the tank battery. During the inspection I met with Mr. Helms and discussed the issues. The tank battery does have secondary containment surrounding a variety of tank sizes and shapes. One of the 140 bbl tanks is empty and according to Mr. Helms will be removed from the system. He did have a recent spill which he attributed to valve failure that released oil into the dike area. Product recovery has utilized a small sump. The recovered oil has been placed in a variety of bottles, buckets and other containers which are all stored inside of the containment area. I advised him to empty all of these small containers into one of the stock tanks and clean up the site. He also needs to post the required identification at the well and the tank battery. Mr. Helms plans place the well into commercial production. In order to do that he will have to post a $5,000 bond and obtain a $3 million liability insurance policy.
