1011161971 2005-11-28 Production Wells Violations Noted The Uniontown office received a call from Jim Erwin with OEPA reporting oil in a creek behind 2449 Greensburg Road. The spill was reported to OEPA by Derek Hupp (330-212-2081). This release is a continuation of the surface spill which occurred on November 10, 2005, near the wellhead. It appears that oil from the surface spill entered clay field tile and drained into a ditch south of the well. The oil traveled due west in the ditch before being contained behind a submerged culvert which acted as a siphon dam. The amount of oil appears to be less than two barrels. I contacted Jeff Murrey with North Coast and informed him of the problem. The pumper responded with sorbent booms and pads. Additional personnel will be on site tomorrow to begin recovery and reclamation operations.

1132584834 2005-11-10 Production Wells Violations Noted The Uniontown office received a call from Jeff Murrey with North Coast Energy reporting an oil spill near the North Canton Transfer #1 well (P/N 153-2710) in Green. The ball valve on the 8-5/8 wellhead was open and it appears that the Oriskany unloaded approximately 30 to 35 barrels of oil and gas through this valve. The affected area extended approximately 100 feet east of the wellhead. On this day, North Coast and Pander Trucking personnel were on site collecting and recovering oil, either with vacuum truck or sorbent pads. Arrangements were made with D&M Contracting to provide a roll-off box and truck to transport the impacted soil to the nearest disposal facility. A north Coast backhoe would be used to scrape the surface and load the roll-off boxes. The North Coast response has been appropriate. On site personnel were advised to continue to remove and dispose impacted soil and to spread peat over the affected area after removal.

Three roll-off boxes had already been removed and there was enough material stockpiled on site to fill two more. After removal is complete, North Coast will spread peat over the affected area. They are also connecting the annulus to the flowline.
