0420992556 2016-08-02 Plug / Plug Back No Violations I was on location at 6:50am with Nicks Well Plugging (NWP), Appalachian Well Surveys (AWS), and Ralph Gregory (Bass Energy) to witness plugging operations on this Clinton Sandstone well. AWS shot off the 4.5 production casing at a depth of 2150. NWP set a 300 plug to isolate the Big Lime and a 200 plug at he bottom of the surface casing. AWS set a steel bridge plug at a depth of 210 and NWP filled the 8.625 surface casing with cement from the top of the bridge plug to ground level. The hole was topped off and the casing will be cut off and fitted with the proper ID plate tomorrow (8/3/16). See plugging report for more details on this job. No violations.
0993273798 2016-07-29 UrbanProduction Wells No Violations I conducted a status check inspection of this directional, Clinton Sandstone well prior to the commencement of plugging operations scheduled for next week beginning 8/1/2016. My inspection found the wellhead caged with screened, chain link fencing equipped with a plunger lift system. A gauge indicated 73psi on the flow line and the valve on the annulus between the 8.625 surface casing and the 4.5 production casing was closed. The man door was locked and the proper ID and warning signage was posted on the fencing. The tank battery was constructed 5-6 below the grade of the parking lot and was fenced and screened. The battery contained one (1) 140 barrel tank, a 50 barrel poly tank, and a horizontal separator. The battery was secured with locks and the proper ID and warning signage was posted as required. No violations.

1895120294 2016-08-08 Plug / Plug Back No Violations The cement top after the plugging and abandonment could not be seen due to restoration. The are of disturbance had been covered over with dirt and gravel. The area will be covered with asphalt per the Landowner. The tank battery was removed at the time of my inspection. A final restoration inspection will be conducted at a later date to check for inadequacies. No violations at thus time.

2057807167 2018-05-16 Final Restoration No Violations The wellhead location was paved over and was being utilized as a parking area for the Canal Place complex. The access for the wellhead and tank battery used previously existing roads for the complex and remain. The tank battery was removed and paved. I did not find any surface expression of previous oil and gas activities. The flow line was reported to be purged with fresh water. The Owners of Canal Place have submitted a Landowner Waiver to permit the flow line to remain once approved. The Canal Place # 2-D met the criteria for Final Restoration. Final Restoration was passed.
2103693891 2016-08-01 Plug / Plug Back No Violations I was on location at 7:45am with Nicks Well Plugging (NWP) and Ralph Gregory (Bass Energy) to witness plugging operations on this Clinton Sandstone well. Gas pressure in the annulus between the 8.625 surface casing and

the 4.5 production casing was blown down for 30 minutes to the tank battery prior to unpacking the wellhead. The tubing in the hole was at a depth of 3849 and NWP ran 3 more joints to a depth of 3945 to get below the bottom perforation at 3940. Due to a large show of oil and sour gas in the Newburg Dolomite during drilling operations, Bass decided to run extra cement on the bottom plug to isolate to the top of the Newburg. NWP rigged up to the tubing, loaded the hole, and broke circulation with 30 barrels of water. They then mixed and pumped eight (8) barrels of cement slurry and displaced the slurry to 3445 with 8.7 barrels of salt water. NWP pulled the tubing from the hole and shut down for the day. See plugging report for more details on this job. No violations.
