0477992339 2006-12-19 Plug / Plug Back No Violations Medina Supply was on site with 3 cubic yards of 9 sack grout to top off the surface plug on this well. The surface hole was originally plugged with the drilling rig on September 3. The grout filled the casing and stayed up. Attached is a copy of the Medina Supply ticket.
1158065163 2006-09-05 Drill / Deepen / Reopen No Violations Surface casing was set at 347 on August 31, 2006. On September 1, Appalachian went back to drilling and encountered more water, sand and gravel under surface casing. Attempts to dry up the hole and continue drilling were unsuccessful and the decision was made to squeeze cement below the casing on September 2. On September 3, Appalachian drilled through the squeeze and water continued to flow. At this time, Schreiner decided to plug and abandon the well and to skid the rig 200 feet to the east. The new location will require a new permit. The well was plugged on September 3. The plug job was not witnessed. Cement reportedly circulated to surface but when inspected on September 5, the plug had fallen approximately 150 feet. I spoke with Schreiner about topping off the plug cement.
