1007934075 2017-01-26 Storage Well No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were closed and locked. The well was fenced and locked. The tank battery still contained production equipment and consisted of one 100 BBL production tank and a mechanical separator. The valves on the separator were closed. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the tank battery. Lease access was stable. No violations at this time.
1192474665 2007-10-15 Drill / Deepen / Reopen Violations Noted On 10/14 at 19:00, I received a call from Mike Reischman at 7111 Taylor Road reporting a soapy film on his water supply. Mr. Reischman (330-351-6323 cell or 330-882-4007 home) obtains his water from a spring that originates southeast of the Kornish well. The spring source rock could have been encountered while drilling the conductor hole. The conductor hole was started in fractured sandstone and there was no circulation while cementing the conductor casing. On 10/15, I inspected the site and found that the spring forms a creek which runs from north to south directly east of Mr. Reischmans house. The domestic water supply is collected where the spring emerges, transfers to a 500 gallon poly tank via a 2 black plastic pipe, then to a 40(?) gallon tank where it flows to either the house system or to a pump house. The excess water from the pump house is discharged back into the creek. I collected water samples from the house, the pump house and from the creek for quantab analysis. The results indicated that chloride content in the house and the pump house is approximately 1000 ppm. The chloride content in the creek is 200 ppm. It appears that the highest concentrations have passed through the creek but remain in the closed system. The plan at this time is to 1) supply Mr. Reischman with a temporary potable water supply; 2) purge the closed system, including the house and pump house; 3) solidify and close the drilling pits on 10/17 and 4) monitor the water quality.
1192714338 2007-10-16 Drill / Deepen / Reopen No Violations Mr. Reischmans temporary potable water supply has been installed and the house has been flushed. A quantab analysis of the spring discharge pipe indicated chloride content of 1100 ppm. After further analysis and discussion, it appears that brine may have impacted the spring via the mousehole and/or the rathole. At this time, the drilling pits are not considered to be the contaminant source because there was no apparent loss of pit fluid during or after drilling and the chloride levels are not higher. Regardless, the pits are scheduled to be solidified on 10/17.

According to Mr. Reischman, he has not been able to use the water well for all of his needs. The bacteria levels have not passed the Summit County Health Department standard. Quantab analysis of the spring indicates that the chloride levels have dropped to the 250 to 270 ppm range. It is possible that the spring will be potable before the replacement water well.

1824832111 2007-11-30 Drill / Deepen / Reopen No Violations Met on site with Mike Reischmann (spring owner), Greg Thiel, Phil Zbasnik, Brian Morley (Dominion East Ohio) and Doug Gonzalez to resolve the water supply concerns. Mr. Reischmanns legal council was also present. The decision has been made to drill a potable water well to replace the brine impacted spring. Dominion will pay for the cost of drilling the well and hooking up the house system. The Quantab analysis this day indicates chloride levels are down to approximately 500 ppm. It still appears likely that the impact will continue to remediate and by spring may be back to pre-drilling levels. Dominion plans to collect water samples from the well and the spring after the well is completed and again in March or April.

The tank battery consists of a 140 BBL production tank and a mechanical separator. The valves at the wellhead were open. The low pressure relief valve, spark arrestor and charcoal filter were installed on the 140 BBL production tank. The tank battery was diked. The fencing around the wellhead and tank battery is an 8 foot high chain link fence and topped with three strands of barbed wire. The man doors to the tank battery and well were locked. The well is properly identified with a clear, legibly marked well identification sign. The lease road is stable and drivable. No violations found.
