0001737711 2016-09-16 Urban Drill / Deepen / Reopen No Violations I was on the Sonoco #1 location at 2300 hrs. to observe the lowering and cementing of the 8.625 surface casing. Poulson Drilling (PDC) drilled a 10.75 bore hole to 370 on a fresh water drilling system. The Berea Sandstone was found in the interval ~ 245 to 254 with a small show of gas. PDC lowered eight (8) joints of 8.625, 23 lbs./ft, 8rd, ST&C with three( 3) centralizers afixed to the casing to 336.5. The running of the casing was completed at 0100 hrs. Petroset Cementing Services cement the surface casing with 200 sacks of Class A cement blended with three (3) % CaCl. Mix water temperature was 70 F and pH was 7. The cement slurry was circulated back to surface prior to releasing the plug. Displacement was achiecved utilizing a wood and rubber plug and 20.5 barrels of fresh water. The cement slurry was scaled at 15.0 lbs./ft. I witnessed 21 barrels of cement slurry returned to the working pit. An eight (8) hours wait on cement time was request before removing the plug container. No violations were identified.
0094857875 2016-09-22 Urban Drill / Deepen / Reopen No Violations The Sonoco # 1 well head was damaged after a dozer involved in the rig demobilization drove across the well head with the blade in the down postion. The well was sruck on the packing ring of the 8.625 X 4.5casing head and forced a 8.625 X 18 nipple to pull out of the 1st collar of the 8.625 casing string. The 4.5 casing was bent to the west at a 10 degree angle from ground level. The 4.5 casing tally was reviewed and it was determined that the top joint of 4.5 casing was a ~ 10.0 pup joint. An assessment as to the integrity of the 4.5 casing could not be made until the 4.5 casing was exposed. I requested that Beck Energy Corp. develop a remediation plan and submit to DOGRM for review.
0130646469 2015-06-04 Urban Drill / Deepen / Reopen No Violations I conducted a status check on the progress of the access road building. I was contacted by David Beck informing me that road construction was to start on 6/4/2015. My inspection revealed a dump truck and dozer were on site but no activities were taking place at this time. I spoke with David Beck and informed him that the required urban signage ( was present, but wrong locale ) should have been placed at the intersection of Sonoco driveway and SR 91. Mr. Beck relayed that the sign would be relocated to the proper place. No violations were identified.
0139750788 2017-10-30 UrbanProduction Wells No Violations On 8/21/2017 I conducted an inspection on the Sonoco # 1 well and tank battery. I found four (4) areas of noncompliance. These areas included locks, vegetative growth, erosional controls and a gate on the access road. I spoke with Rick Reeves ( Beck Energy ) and informed of my findings. Mr. Reeves said, that it will be remedied. My inspection today indicated that the non-compliant issues were corrected. No further enforcement actions required.
0223492893 2016-09-21 Urban Drill / Deepen / Reopen No Violations Poulson Drilling Corporation reached the total depth of the Sonoco # 1 well at 4124 KB and was mixing starch into the circulating system to condition the well bore. An inspection of the location did not reveal any violations.
0233453138 2016-09-27 Casing String Witnessed No Violations I was on the Sonoco # 1 location to observe the repairs to the conductor, surface and production casing strings which were damaged after being struck by a bull dozer ( 9/22/2016 ). Moore Well Service (MWS) was the contractor selected to remove the damage sections of casing and repair the same. After removing the cellar from around the well head MWS removed the visible sections of damaged casings. The soil was removed surrounding the well head in a circular pattern 30 in diameter and 10 deep to allow access to cut and remove the conductor, surface and 4.5 production casings. After removing the 11.75 conductor and 8.625 surface casing, the 10 X 4.5 pup joint ( top joint of string ) was removed and replaced. MWS welded a section of 8.625 casing to bring the 8.625 casing up to grade. The excavation was back filled and compacted. A 8.625 X 4.5 casing head was attached to 8.625 casing and MWS utilized a Casing Jack to pull stretch on the 4.5 casing and set the slips. DOGRM will monitor the hydralic pressure testing and Cement Bond Log. No violations were identified during this inspection.
0263981035 2016-09-14 Urban Drill / Deepen / Reopen No Violations I was on the Sonoco #1 location to observe the drilling of the conductor hole. Poulson Drilling was mixing fresh water mud at 0700 hrs. and spudded the well at 0845 hrs. At 1038 hrs. the well bore samples indicated that Poulson was drilling sand and coarse gravel at 35. At 68 the samples indicated the formation drilled was sand and coarse gravel with some grey shale present. Poulson Drilling had to mix more fresh water mud due to the downhole water influx cutting the viscosity of the drilling mud and not cleaning the bore hole. At 84 the samples changed to 90% grey to black shale and 10% coarse gravel. The geologragh showed a distinct drilling break at 89 and the shale bedrock was encountered at 90. Poulson Drilling continued to drill the 14.74 well bore to 127 where the Operator called T.D.. After conditioning the bore hole for 2.5 hrs. Poulson removed the drilling pipe and attempted to lower the 11.75 conductor casing into the bore hole. The conductor casing repeatedly sat down on a ledge at 80 where the formation transitioned from gravel to shale. Poulson Drilling removed the 11.75 conductor casing from the bore hole and lowered their drill collars into the hole to ream and condition the bore hole. Due to the extended time requirement to perform such operations, I left location and waited to be notified. No violations identified during this inspection.
0702214615 2016-09-14 Drill / Deepen / Reopen No Violations I was on location with the Summit County Inspector Bob Roush to witness the start of the drilling the well at this location. Polson Drilling began mixing their drilling mud. At 8:45 AM they started drilling a 16.750 hole at 18-19 they encounter some hard drilling. We were taking drilling samples and comparing with the water well logs from the water well field west of this location. I left location at 5:00 PM at that time they were at 120.
0749854335 2016-09-29 Completion Testing No Violations After the completion of the 8.625 surface casing and 4.5 production casing repairs ( 9/27/2016 ) Beck Energy Corp. pressure tested the 4.5 production casing to demonstrate casing integrity before DOGRM would allow to proceed with completion of the well. Pyramid Well Treating was on location to pressure check the casing with fresh water. After purging the system of air and fixing various leaks, Pyramid pumped approximately two (2) barrels of fresh water and pressurized the 4.5 casing to 2050 psi. and shut the well in. A gauge on the well head was monitored for any pressure decline. The pressure was recorded every five (5) minutes for thirty minutes. The pressure did not decline from 2050 psi. and after thirty minutes the test was passed. No violations identified during this inspection.
0752344297 2016-09-19 Urban Drill / Deepen / Reopen No Violations I was on the Sonoco # 1 location to inspection the drilling activities. Poulson Drilling Corporation was the drilling contractor and was drilling at 3213 on a fluid drilling system. The location was dry and the pits had a minimum of two (2) feet freeboard. A gas detector was in working condition and monitoring the circulating fluid. The average background gas units detected for the previous 24 hours was 17 units. No H2S gas was detected. Total footage drilled in last 24 hours was 708. No violations were identified during my inspection.
0752725349 2016-09-27 Drill / Deepen / Reopen No Violations I was on location to witness the repairs of the well head. The well head was damaged by a dozer when the drilling rig was moving off of the location. Moore Well Service and Palmer Trucking was the contractor on location to repair the damaged wellhead. The 8-5/8 was cut off 10/12 below ground level. There was a 10 4.5 10.5 lb/ft pup joint that was also replaced. The 8-5/8 was welded. MWS used there hydraulic casing jack and pulled 7.250 of stretch on the 4.5 casing and set in the 8-5/8 slips The well was repacked a pressure test will be performed at a later date. Various pictures were taken. Inspector Bob Roush has them.

They were also trenching in one (1) two (2) inch plastic and one (1) one (1) inch plastic line toward the Dominion / EOG meter point on S.R. 91. The two (2) inch line was the gas sales line with the one (1) inch line being a line to blow the drip back to the tank battery. No violations identified during this inspection.

No material will be hauled away today until all liquids are solidified. Daley Excavating constructed a dike around the east end of the location to control the rain water from running in the pits.

1203710962 2016-12-21 UrbanProduction Wells No Violations I inspected the Sonoco # 1 well to assess compliance with 1501:9-9-05 (E) OAC regard to Urban Drilling requirements. The well and tank battery were found to be noncompliant with the following required items: 1.) Access road gate with proper Urban Signage, 2.) Urban Signage requirement not met, 3.) Fencing and Landscaping requirements not met, 4.) Locks on all valves, 5.) removal of debris ( 8.625 casing stump ). The well was not in production at the time of my inspection. I spoke with David Beck ( Beck Energy, 330.297.6964 ) and inform him of the deficiencies I identified. I told David Beck that all the noncompliance items would have to be corrected prior to initiating production.

1400250232 2016-10-25 Surface Facility Construction No Violations I was inspecting the Sonoco # 1 to check on the facilities construction progress. The location was fenced off with safety fence, but the northwest corner have been tampered with by tresspassers gaining access to the Cuyahoga River. The well head was comprised of the 1.5 tubing with a gate valve on top and two (2) ball valves were attached to the tubing head. The wells 4.5 casing pressure was approximately 780 psi. ( per J-Well crew ). I requested from David Beck ( Beck Energy Corp. ) that plugs be tightly screwed into the valves to eliminate any possible vandalism. The silt fencing was still in place and effective. Two (2) 100 barrel stock tanks were on location, but construction of the tank battery had not began. No violations identified during this inspection.
1458210384 2016-09-15 Urban Drill / Deepen / Reopen No Violations I was on the Sonoco # 1 at 0320 hrs. to witness the re-running and cementing the the 11.75 conductor casing after Poulson Drilling reamed and re-conditioned the bore hole. The 11.75 conductor casing was lowered in the bore hole. Three (3) joints of 11.75, 32 lbs./ft, 8rd casing was lowered into the bore hole at 0500 hrs.. One (1) centralizer was positioned at 44. At 0635 hrs. Petroset Cementing Services (PCS) moved in and connected to the conductor casing in preparation to cement the casing. The mix water temperature was 65 F and pH was 7. PCS mixed a cement slurry ( Class A cement with three (3) % CaCl., 340 sacks ) and displaced it back to surface. The slurry weight was 15.2 ppg as verified by scale. I witnessed 12 barrels of cement slurry returned back to surface after the displacement was completed. I requested a wait on cement time of eight (8) hours before removing the head. No violations identified during this inspection.
1474376259 2016-09-15 Drill / Deepen / Reopen No Violations I was on location to witness the construction of the blow pit and spraying the side walls and the bottom of the pit with bentonite gel and lining the pit with two layers of 20 mil pit liners. They pumped 100 barrels of pit mud from the working pit to the blow pit to be removed. Location is clear of all debris. Silt fence is in stable condition. All pits are stable. ID signs are posted.
1474383379 2016-09-09 Drill / Deepen / Reopen No Violations I was on location for a per-spud meeting with Summit County Inspector Bob Roush, Ray Beck, David Beck from Beck Energy and Roy Polson from Polson Drilling and Kelly Daley from Daley Evacuating. The drilling permit and conditions were discussed. The cementing procedure for the conductor and surface casing was discussed The water tight cellar was also installed at that time.
1474642734 2016-09-16 Urban Drill / Deepen / Reopen No Violations I was on location to witness the pressure testing of the annular Blowout Preventor ( BOP ) prior to drilling deeper out from the surface casing. Poulson Drilling ( PDC) employees had rigged up the BOP and prepared to start the test. I requested a minimum pressure of 300 psi. PDC closed the annular BOP, with nitrogen, on 6.5 drill collars and used the mud pump to increase the 8.625 casing pressure. After reaching approximately 300 psi. it was evident that there was a leak in the system by the pressure decline. An examination of the equipment indicated that a short nipple, on which the BOP sat, had developed a crack and would require a repair. As it was determined that the repair would require a few hours to effect I left the location and waited to be recalled for the retesting.

1475159294 2016-09-23 Drill / Deepen / Reopen No Violations Diamond Energy Services was on location removing drilling fluids and disposing the fluids at there disposal well. Daley Excavating was moving equipment on location to prepare to solidify the pits. The drilling rig and equipment has moved. I walked the location it is clean of debris. The well head was damaged during the rig move, Plans are in the process to repair the well head.
1475687376 2016-09-21 Casing String Witnessed No Violations I was on the Sonoco # 1 drilling site to witness the running and cementing of the 4.5 production casing. Poulson Drilling Corp. (PDC) drilled a 7.875 bore hole, on fluid, to 4124 KB. The 4.5 production casing was lowered to 4070 KB. Petroset Cementing Services (PCS) connected to the 4.5 casing and conditioned the bore hole with 10 barrels of gelled fluid and then mixed 175 sacks of Class A cement blended with 5 lbs. / sack gypsum. The mix water temperature was 68 F and pH was ~7. The cement slurry was displaced with a wiper plug and 63 barrels of fresh water. The cement slurry weight was verified, by scale, at 15.0 ppg. After the plug was landed, 1500 psi was held on the 4.5 casing for the green cement test for five (5) minutes without any pressure decline. The 4.5 production casing was centralized with 12 bow spring centralizers and casing hardware included guide shoe, latch down baffle and plug. The pits had the required free board. No violations identified during this inspection.
1476466560 2016-10-04 Completion Testing No Violations After the completion of the 4.5 casing repairs, I was on the Sonoco # 1 location to observe Appalachian Well Surveys (AWS) run a Gamma Ray / CCL / VDL Cement Bond Log to determine if the primary cement job was sufficent for zonal isolation. AWS lowered their tools to 4065 and logged up to 3200. The cement top was found to at ~ 3382. The cement sheath was found sufficent for zonal isolation and to allow Beck Energy Corp. to stimulate the Clinton sandstone. No violations were identified during this inspection.
1479141142 2016-11-14 UrbanProduction Wells No Violations An inspection of the Sonoco # 1 revealed that Daley Excavating personnel were on location to construct the well head and tank battery. The well head was built to operate as a plunger lift well. The manifold to monitor the 8.625 annulus was constructed and functional. I did not detect any leaks at the well head. The tank battery was comprised of two (2) 100 barrel stock tanks, two (2) 30 barrel poly brine tanks, a 1810 horizontal separator and a gas measurement station. The tank battery was surrounded with a earthen dike. All equipment spacing rules were observed and found to be correctly positioned. The lightening arrestor, pressure relief valve, charcoal filter and pressure relief inspection hatches were on location, but not installed at the time of this inspection. I did not identfy any violations during this inspection.
1501116521 2016-09-22 Urban Drill / Deepen / Reopen No Violations I was on the Sonoco # 1 location to access the damage caused by a bull dozer striking the well head and bending it over almost parallel to the ground. I saw the 4.5 casing bent over to within ~ 18 of the ground leveland the 8.625 surface casing had a 18 X 8.625 nipple pulled out of the first 8.625 collar. There was not any releases ( gaseous or liquid ) identified. The 4.5 casing had just been cemented approximately 12 hours earlier. Beck Energy Corp. decided to wait another 24 hours to allow the cement sheath to cure. I informed Beck EnergyCorp. That a remediation plan would be submitted and approved to DOGRM with testing prior to any repairs.
1503320353 2017-08-21 Preliminary Restoration No Violations The well was accessed through the northeast corner of the parking lot at Sonoco Paper Products. A gravel road provided adequate entrance to the well and tank battery. The well was operated as a plunger lift well. A locked, screened fence ( eight foot with barbed wire ) provided security. The annular pop-off valve and gauge were in place. The annular gauge read zero psi. The tank battery was surrounded with an earthen dike and an eight foot locked, screened fence topped with three strands of barbed wire. The tank battery consisted of two 100 barrel stock tanks, two 35 barrel poly brine tanks, a two foot by forty foot volume tank, a 1810 horizontal separator and gas measurement station. The stock tanks had lightning arrestors, pressure sealing hatches and a charcoal filter for the vent lines. The required urban signage was in place and legible. The areas of previous disturbances were vegetated and stable except where noted in the following. I found several deficiencies including; absence of locks on the load lines, an area north and west of the tank battery where reseeding is required, re-establishment of the silt fencing west of the tank battery and placement of a gate where the access road leaves the parking lot. I spoke with Beck Energy office receptionist, gave a list of the deficiencies and requested that David Beck call me for discussion.
1646097362 2016-09-13 Urban Drill / Deepen / Reopen No Violations I was on the Sonoco # 1 location to observe the construction of the working pit. Daley Excavating was the dirt contractor for the well. Daley Excavating dug the flow ditch from the well cellar and lined it with a liner. Daley then constructed a three (3) section working pit of approximately 40 X 105 size. I requested that the angle of repose be decreased because of the unconsolidated nature of the sediments. Poulson Drilling employees coated the walls and bottom of the working pit with 1000 lbs. of bentonite gel. A 20 mil pit liner was installed in the working pit. The lined pit was filled with city water from the Sonoco plant. No violations were identified during this inspection.
1820087094 2016-09-13 Drill / Deepen / Reopen No Violations I was on location with Summit County Inspector Bob Roush to observe Polson Drilling moving and setting up their drilling and the construction of their working pit. All silt fencing was installed and drilling location was also fenced around the drilling pad perimeter. Drilling permit and conditions were posted in the dog house.
1988014266 2007-12-17 Drill / Deepen / Reopen No Violations Columbus requested a status check inspection at this site. There has been no activity to indicate that drilling will occur in the near future. Neither the location nor the lease road have been built.
