The tank battery consists of a 140 BBL production tank and a mechanical separator. The valves at the wellhead were open. The low pressure relief valve, spark arrestor and charcoal filter were installed on the 140 BBL production tank. The tank battery was diked. The fencing around the wellhead and tank battery is an 8 foot high chain link fence and topped with three strands of barbed wire. The man doors to the tank battery and well were locked. The well is properly identified with a clear, legibly marked well identification sign. The lease road is stable and drivable. No violations found.

1528614470 2017-01-26 Storage Well No Violations At the time of this inspection, the well was equipped with a plunger lift assembly and the valves on the wellhead were closed and locked. The well was fenced and locked. The tank battery still contained production equipment and consisted of one 100 BBL production tank, one 50 BBL poly brine tank, a mechanical separator. The valves on the separator were closed. The tank battery was diked and fenced with a locked man door. The proper, legible signage was located at the tank battery. Lease access was stable. No violations at this time.

1976177959 2011-04-05 Urban Drill / Deepen / Reopen No Violations During the Universal frac job on 3/30, the 4-1/2 casing failed just below the top collar. The top joint was a pup which may or may not have been cut from the same pipe. The casing is 11.6# and was cemented back to surface on 3/17 with 65 bbl returns. According to Don Kreager representing East Ohio, the job was approximately 70% complete and Universal was blending and pumping sand. It appears that the failure occurred in the collar and/or pipe threads. After the failure, the well started flowing back and the returning frac sand started cutting the casing. The well was allowed to blow down until it could safely be diverted to a frac tank. Some water and sand was released onto the location and a retention area was built to contain any lost material. According to Kreager, approximately 700 bbl of surface runoff, stormwater and possible frac flowback were pumped into empty 300 bbl frac tanks for later disposal. On this date, Baker Atlas was on site to run cased hole logs to evaluate the 4-1/2 casing condition. Dominion plans to remove and replace the 4-1/2 pup joint either by backing off, if possible or cutting off if not. This activity will occur in the next several days.
