1457968246 2016-03-14 UrbanProduction Wells No Violations This inspection ( 03/14/2016 ) was conducted to access the compliance with a previously issued Compliance Notice ( # 18264471477 ). The prior deficiences included I.D. signage, locks on load lines and a pop off valve on the 8.625 to 4.5 annular space. My inspection indicated that all item noted previously have been brought into compliance. No further violations noted.
1933104074 2014-12-08 UrbanProduction Wells Violations Noted I inspected this well on 12/8/2014. The lease road is graveled and passable. All the facilities ( wellhead and tank battery ) are fenced. The well was operated as a pumping well utilizing an electrified Linear Rod Pump. The construction of the well has 2.375 upset tubing inside 4.5 casing. All piping and fittings were leak free at the wellhead. The pop-off on the 8.625 casing was lacking the required pressure gauge. The tank battery consisted of a 210 barrel oil tank, 100 barrel brine tank and a hornizontal mechanical separator. The load lines were equipped with flat plugs, but were not locked. The require urban signage was absent. There were non-essential items ( frac valve, safety fence, etc. ) that need to be removed from the tank battery.In order to bring the Loyal Oaks Swim Club # 1 into compliance the Operator shall, 1.) Place the required Urban signage at the well and tank battery, 2.) Place locks on the load lines of all tanks, 3.) Place the required gauge on the 8.625 pop-off manifold and expose the valves, 4.) remove all non-essential items ( frac valve, safety fence, etc. ) from the tank battery. These corrective measure shall be completed and approved ( by DOGRM Inspector ) by 1/23/2015
