0022755859 2017-09-28 Preliminary Restoration No Violations Access was through a gate at the end of Piedmont Street. Urban signage was posted at the gate. The lease road was graveled to the wellhead and tank battery. The previously disturbed area was vegetated and stabilized. I did not see any erosional features on location. The well was operated as an electric pumping unit. The wellhead and pump jack were surrounded with locked board on board fencing. Urban compliant signage was posted. The electric contol panel was locked. The stuffing box required maintenance at the time of this inspection. I spoke with Ralph Gregory ( Bass Energy 330.620.5857 ) about perform the required repairs and cleaning up the wellhead. Mr. Gregory said, he will have the work done asap. I will re-inspect to assure it is completed. The annular pressure gauge read 75 psi ( allowable pressure at the casing seat was 131 psi. The tank battery was surrounded with an earthen dike and board on board locked fence. The signage was Urban compliant. Equipment comprising the tank battery included one (1) - 140 barrel stock tank, one (1) - 50 barrel steel brine tank, one (1) - 2408 separator and one (1) - DOM / EOG metering station. The stock tank was urban compliant with charcoal filter, low pressure relief valve, lightning arrestor and sealing inspection hatch. The load lines were locked and plugged. A dike drain was installed with a closed valve. I did not identify any leaks or contamination in the tank battery. The Hardy Unit #1 was passed for preliminary restoration.
0886128373 2014-03-11 UrbanProduction Wells No Violations I inspected this well and tank battery on 3/11/2014. I met with Ralph Gregory on location to discuss the annular pressure I observed. The gauge initially read 680 psi, which exceeded the allow annular pressure of 131 psi with a casing seat of 432 KB. I asked for a new gauge to be used for a reading. The new gauge read an annular pressure of 60 psi, which is within the allowable limit. During wellhead construction the required Pop - Off valve was not installed. I requested that the missing valve be put on the 8.625 annulus within a week ( 3/18/2014 ) if it is not a Compliance Notice will be issued. The tank battery consists of a 140 Bbl. Stock tank, 50 Bbl. Steel brine tank, a 2408 Westerman separator with gas measurement station. The dike is adequate to contains all fluids of the tank battery, the required spacing between the stock tank and separator is present, the loadlines were locked and had flat plugs tightened. The lease road is graveled and passable. All the required identification signage was properly placed and legible.
1535631887 2018-08-29 Preliminary Restoration No Violations Access was on a locked gated graveled road to the tank battery and wellhead. The tank battery was surrounded with a locked board on board fence and an earthen dike. Urbanized I. D. signage was posted at the tank battery. The tank battery was comprised of one (1) 140 barrel stock tank, one (1) 50 barrel steel brine tank, one (1) 2408 horizontal separator and one (1) Dominion / EOG rotary gas meter. The load lines were plugged and locked. A carbon filter, pressure relief valve, lightning arrestor and sealing hatch were on the stock tank. No leak were found at the tank battery. The well was operated as an electric pumping well and was surrounded with a locked board on board fence. The electric panel was locked. Compliant Urbanized signage was posted on the fence. The well tender was present during my inspection and the well was in operation. The well tender was cleaning the wellhead after the second yoke on the stuffing box had leaked. The stuffing box was re-tightened to prevent further leakage. I did not see any free oil on the ground. The oil and grease covered the wellhead down to the 8.625 casing head. The well tender was in the process of wiping down the wellhead. I requested that the stuffing box be examined and maintained regularly to prevent further leakage. I will re-inspect to evaluate whether further enforcement is required. The location was vegetated and free from erosional features. Preliminary Restoration was passed.
