1473170007 2016-07-19 Work Over Reconditioning No Violations I was on location to observe the re-frac of the Goodview Park # 1-D. Producers Service Corp. was the stimulation contractor. There was not a noticeable Breakdown pressure observed. The well treated at 1720 psi @ 12.5 bpm. A total of 150 sacks of 20/50 sand were used. Average treating pressure was 1664 psi. Total job volume was 34,000 gallons of fluid. Five (5) minute shut in pressure was 1406 psi. No violations were identified.
1541651276 2016-07-20 Work Over Reconditioning No Violations I conducted a status check on this location after the frac job. Bishop Well Services was sand pumping the 4.5 casings rathole. Very little ( less than five (5) gallon bucket ) material and sand was recovered from the well. There was not any pressure or shows of oil. No violations identified .
1883062388 2016-10-17 UrbanProduction Wells No Violations The Goodview Park Unit # 1 - D well is operated as an electric pumping unit. The access to the well head was gained by using a grass path as the well was located on park land. The well had a ten ( 10 ) foot fence topped with five ( 5 ) strands of barbed wire. The I.D. signage had the proper information and was of the required letter size. I requested that the sign be relocated to allow visibility from the street. The fencing was locked. The well head was configured as a typical pumping well head. The 8.625 riser was present, but lacked the required gauge. The electric box did not have the required lock. Both these inadequacies were address with Operator. The tank battery was located in an industrial park, which afforded sufficent access. The tank battery consisted of two ( 2 ) 210 barrel stock tanks, one ( 1 ) 100 barrel steel brine tank, two ( 2 ) 25 barrel poly brine tanks and two ( 2 ) horizontal ( 2408 ) mechanical separators. The stock tanks were equipped with charcoal filters, lightening arrestors and pressurized inspection hatches. The required spacing was observed between the tank battery equipment. The load lines were locked and plugged. The tank battery was surrounded with a ten ( 10 ) foot fence topped with five ( 5 ) strans of barbed wire and one ( 1 ) strand of concertina ribbon. An earthen dike surrounded the tank battery. I will re - inspect the pumping unit to ensure the inadequencies previous mentioned are address.
2047044130 2016-06-28 Work Over Reconditioning No Violations I inspected the Goodview Park well # 1-D to review the preparations for a remedial polymer treatment to control the excessive production water production. The Operator decided to treat the well with a polymer fluid inorder to control excessive water production and refrac the well. On location was a 300 barrel fresh water tank ( filled with hydrant water), enclosed pumping and chemical equipment trailers and a generator set. The location was surrounded with silt filter sock on the southern border. At the time of my visit Flotek, Ind. was pumping treatment fluids downhole at a rate of 1.5 bpm with a pressure of 680 psi. down the 2.375 tubing. No violations were observed during this inspection.
