1523981190 2013-12-05 Preliminary Restoration No Violations I inspected this location for preliminary restoration and proper spacing of the Tank Battery. Upon my inspection I observed that all the criteria was met for the preliminary restoration. Pits were filled and all disturbed area were stabilized and free of any erosion. Erosional controls were in place and functional. Upon examination of the Tank Battery I found all spacing requirements were complied with. The Wellhead was measured and found to be 61 from the 140 Bbl. Stock tank. Pressures at the Wellhead were 1000 psi on the 4.5 casing, 720 psi on the 1.5 tubing and 0 psi on the 8.625 to 4.5 annulus. I did not identify any violations at this inspection.
1897132364 2016-01-26 Preliminary Restoration No Violations I inspected the Donzelli #1 to confirm compliance with the Preliminary Restoration criteria. All criteria for Preliminary Restoration were met. During my inspection Ohio Valley was swabbing the well. No personnel were on site. I call Rick Liddle ( OVE ) to inform him that during the swabbing operation the crew had swabbed some oil onto the top of the tank and it was running down the front of the 140 barrel storage tank. I requested that when swabbing operations are completed that the tank be cleaned off and that the inspection hatch be latch down tight.
1956777455 2016-11-18 UrbanProduction Wells No Violations Due to the high fluid production, the Operator decided to convert the production technique from a plunger lift well to the pumping well. They requested to be allowed to utilize a natural gas powered motor for a 30 day evaluation period due to the expense of running electric service back to the well. If the well warrants permanent conversion, the prime mover will be switched to electric. Since the well location is located in a parking lot (Donzells Nursery) and sparsely populated ( closest house 350 to 400 south ) the request was allowed, but if any complaints ensued the evaluation would cease. The well was set to pump five (5) days a week from 7:30 am to 6:45 pm. My inspection did not detect excessive ambient noise coming from the pumping unit. I requested that the Operator placed the required urban signage on the well. The present signage did not meet the Urban requirements.
1969429093 2018-12-28 UrbanProduction Wells No Violations The well was converted to a pumping well. The wellhead was surrounded with a locked chain link fence with privacy slats and landscaping. The tank battery consisted of one (1) 140 barrel stock tank, one (1) 50 barrel poly brine tank and one 2408 horizontal separator. The 140 barrel tank was compliant with SB 165. A chain linked fence with privacy slats and an earthen dike surrounded the tank battery. Landscaping offered an effective screen. Legible I. D. signage was posted. The load lines were locked and plugged. No leaks were found.
