0597095676 2017-04-20 UrbanProduction Wells No Violations I was on location as a request from the landowner, Jessica Irwin concerning the new fencing around the tank battery. I found two (2) of the new fence panels have come loose from the posts making the tank battery unsecure. I spoke with O & M Enterprises. I told them to repair the fence panels. A follow-up inspection will be conducted by 28 April 2017.
0649462596 2012-10-03 UrbanProduction Wells No Violations I conducted an urban production well inspection status check. My inspection found the well equipped with a plunger lift unit with no identification and surrounded by a board-on-board fence that was missing slats and the gate was unhinged. The tank battery was surrounded by a board-on-board fence that was missing slats and no lock on the gate. There was one, 100 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and the proper identificaiton. The production equipment had the proper spacing. The lease road was in good condition. I researched our database and found there has been no reported production. I called O & M Enterprises at 330-393-3200 and asked for all of the compliance violations to be fixed, send the production records to Columbus, or file for a plugging permit. I will perform a follow-up inspection by 10/18/2012.

I noticed the board-on-board fences had been repaired, the fence gate for the plunger lift unit had been replaced, and new locks installed.

O&M Enterprises has one month to: Repair the leaking fitting on the flow line at the well. Update the identification sign on the 100 bbl. storage tank. Place an identification sign on the tank battery fence. Repair the east side of the fence at the tank battery. Replace/repair all of the missing, broken, and fallen wooden slats for the fences around the tank battery and well. Repair the latch on the fence man-door at the well. Install locks on the load lines for the two storage tanks.

1483540460 2017-01-03 UrbanProduction Wells No Violations I was on location to assess complaint #5475 and compliance notice # 1479915464. At the time of my inspection, I found new fencing built around the tank battery and well. Both fences are locked. The identification sign at the tank battery has been updated. Locks have been placed on the load line for the 100 bbl. storage tank. The leaking fitting on the flow line at the well has been repaired. I spoke with complainant, Mr. Erwin concerning the fencing and repairs. There are no further violations at this time.
1494817064 2015-02-23 UrbanProduction Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well was equipped with a plunger lift unit. The tank battery consisted of a 100 barrel production tank, a 50 barrel brine tank and a mechanical separator. A dike surrounded the tank battery. An identification sign were posted at the tank battery. There was a plug on the load line. A production meter and 7 day chart was installed inside the separator. Wooden fences surrounded the well and tank battery. The lease road was in stable condition. The well was placed into production prior to House Bill 278.
1502372990 2016-12-23 UrbanProduction Wells No Violations I was on location to assess complaint #5475 and compliance notice #1479915464. At the time of my inspection, I found an O&M Enterprise crew on location building a new wooden fence around the tank battery. All of the old fencing around the well and tank battery has been removed. O&M Enterprise expects to have the new fencing around the tank battery and well completed by 12/28/2016.
1521721913 2018-01-16 UrbanProduction Wells No Violations I conducted an inspection of this well site as a request from the landowner, Jessica Irwin, regarding the fencing around the tank battery. During this inspection, one of the fence panels had fallen over and needed reattached to the fence post. I contacted Bob Burkee, owner of O & M Enterprises and discussed repairing the tank battery fence. Mr. Burkee stated he would send out the field supervisor to assess and remedy the situation.
