0832918900 2015-02-10 UrbanProduction Wells No Violations I was on location for a follow-up inspection. During my inspection, I found the 100 barrel production tank removed from the dike. All of the soil and snow impacted by the oil spill was removed. I spoke with Richard Helderman of NCL. Mr. Helderman stated new gravel would be placed in the dike followed by a new 100 barrel production tank. Also, Mr. Helderman stated the leak originated from the bottom seam of the tank.
1339435171 2012-06-07 Production Wells No Violations On 6/7/2012, I completed a production well inspection status check. This well shares a lease road with permit number 2-3358. My inspection found the well was a plunger lift unit (rabbit) that was in good condition. The tank battery contained one 100 barrel oil storage tank, with the proper identification posted, and a separator. I noticed no visible spills or leaks. The lease road was in good condition, there are no violations at this time.
1422372240 2015-01-27 UrbanProduction Wells No Violations I received a called from Richard Helderman, field supervisor for NCL on 1/26/2015 around 4 pm concerning an oil spill. Mr. Helderman stated approximately 16 bbls of brine and crude oil was released from the production tank and contained within the dike. Mr. Helderman stated clean--up actions. Mr. Helderman stated:


-3358, which shares the lease road.



I instructed Mr. Helderman to:

-up operations.

-up is complete.

1500342730 2015-02-25 UrbanProduction Wells No Violations I was on location for a follow-up inspection. During my inspection, I found the oil spill completely cleaned up. All impacted material had been removed. Clean gravel had been placed throughout the dike. A new, 100 barrel production tank had been placed on three (3), 6x6 boards. An identification was posted.
