0403908431 2014-11-17 Final Restoration No Violations I was on location for a final restoration. During my inspection, I found all of the production equipment was removed. The site was graded and vegetation was established. All debris was removed. Pits were not used during the plugging of the well. There was no lease road to restore; well was located next to a factory and access to the well was paved. Final restoration passed.
0833902868 2013-08-08 Plug / Plug Back No Violations I witnessed Nicks Well Plugging set the bottom plug for NCL. Nicks operator pumped on the well with 100 bbls of fresh water to circulate the rabbit out of the plunger lift unit. Next, 149 joints of 2.375 tubing were removed. The operator established circulation with fresh water. Nicks Well Plugging mixed and bull headed 25 sacks of Class A cement with 50 lbs. of Hulls. The well was shut-in at 1,500 psi.

1514430331 2013-08-09 Plug / Plug Back No Violations Nicks Well Service ripped the 4.5 production casing at 3,254, then pulled 7 joints to set the Big Lime plug. The operator established circulation with 12 bbls of salt water, then pumped 86 sacks of Class A cement. The cement was displaced with approximately 43 bbls of salt water. Plug #2 was set from 2,690 to 2,990. Nick pulled 58 joints of casing to set the surface casing plug. Circulation was established with 15 bbls of fresh water, followed by 60 sacks of Class A cement. The cement was displaced with 5 bbls of fresh water. Plug #3 was set from 302 to 502. The operator pulled 7 joints of casing to set the surface plug. The operator established circulation and pumped 62 sacks of Class A cement. I witnessed a good cement returned to surface. The operator removed the final 5 joints and the cement was topped off at the surface. The cement was static at surface after 20 minutes. A total of 77 joints of 4.5 production casing and 149 joints of 2 tubing was removed from the well.
