0471480299 2019-03-08 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 1130044330, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, the 210 BBL storage tanks now have installed the required urban safety equipment. The wooden fence surrounding the tank battery has no way to secure the man door and needs to be secured and locked. The areas of the wooden fence that previously needed repaired have been repaired. The identification sign has been amended with the required information per 1501:9-9-05(E)(1)(a-e). The access gate was present but no identification has been installed on the gate.

PAC needs to: 1) Install signage at the lease gate per 1501:9-9-05(E)(1)(a-i). 2) Install a lock on the tank battery fence per 1501:9-9-05(E)(3).

On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. An agreement has been made with PAC Drilling to complete one (1) to three (3) equipment installations per quarter to place the wells in compliance with 1501:9-9-05(E)(4).

0728281456 2017-05-24 UrbanProduction Wells No Violations I found the flow line leak between the tank battery and the well repaired on the day of my inspection. All of the impacted soil and vegetation has been removed. The area is graded and seeded.
1130044330 2018-05-14 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly and the annulus and flow line valves were open. A locked wooden fence surrounds the well with an identification posted on it. The diked tank battery has three storage tanks and a separator. The pick-up lines have plugs and the valve handles have been removed. The 210 BBL storage tanks are missing the required urban safety equipment. There was an identification sign posted on the wooden fence at the tank battery, but no lock on the fence. Parts of the tank battery fence need repaired and are falling down. The identification sign was missing the required information per 1501:9-9-05(E)(1)(a-e). The tank battery was shared with wells 2-3951 and 2-3949. The access was stable with a closed gate but no signage on the gate.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 3) Install signage at the lease gate per 1501:9-9-05(E)(1)(a-i). 4) Install a lock on the tank battery fence per 1501:9-9-05(E)(3).

1401131195 2012-09-05 UrbanPreliminary Restoration No Violations I completed an Urban Preliminary Restoration inspection. My inspection found the well surrounded by a board-on-board fence and the proper identification posted. The tank battery was surrounded by a board-on-board fence and contained two 210 barrel storage tanks, one 50 barrel brine storage tank, the proper identification posted, and a separator. The production equipment had the proper spacing. The site has been graded and seeded to prevent erosion and sedimentation. This well shares a lease road and tank battery with permit numbers 2-3949 and 2-3951. Inspection passed.
1448339721 2016-03-07 UrbanProduction Wells No Violations I was on location in reference to compliance notice # -2033984734. During my inspection, I found a new lock installed on the fences surrounding the tank battery and well. There are no further violations at this time.

--up line has a plug, but no lock. The 210 bbl. storage tanks are missing the required urban safety equipment. There is an identification sign posted on the fence at the tank battery. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a--in the well and stated the leak would be repaired and cleaned. Photos were taken and are available upon request. A follow-up inspection will be conducted by 28 April 2017.

2033984734 2016-01-21 UrbanProduction Wells Violations Noted I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. There is a wooden fence with no lock around the well. The tank battery has two 210 barrels production tanks, a 50 barrel poly brine tank, and a mechanical separator. A dike surrounds the tank battery. Identification signs are posted at the tank battery and well. A wooden fence with no lock surrounds the tank battery. The lease road is in stable condition with a locked gate at the entrance. The tank battery is shared with permit number 2-3949 and 2-3951. I spoke with Rusty Hornhill, PACs well tender. I told Mr. Hornhill a compliance notice would be issued for failure to secure the fence around the tank battery and well with locks. I told Mr. Hornhill a follow-up inspection would be conducted by February 8, 2016.
