0479156434 2012-11-30 UrbanPreliminary Restoration No Violations I completed an urban preliminary restoration inspection. My inspection found the well equipped with a plunger lift unit. The tank battery was shared by permit numbers 2-3764, 2-3778, and 2-3821 and contained one, 100 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, a volume bottle, and an identification sign. The lease road was part of the maintenance area of the golf course. I noticed no visible spills or leaks. The pits were closed and debris removed. The site was graded and vegetated.
0669013988 2016-11-27 UrbanProduction Wells No Violations At time of my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The diked tank battery has two storage tanks, a separator and a volume bottle. Identification signs are posted at the well and tank battery. The load line is secure with a plug. The access road and production equipment are in good repair. The tank battery and access road are shared with permits 2-3821, 2-3764 and 2-3778.

The equipment on the site consists of the following:



1447184359 2015-11-03 UrbanProduction Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The 2 flow line ball valve at the well is in the open position. The tank battery consists of a 100 barrel production tank, a 50 barrel brine tank, and a mechanical separator. There is a dike around the tank battery; however, the north end of the dike is completed weathered and in capable of meeting containment. There are identification signs posted at the well and tank battery. The identification sign at the tank battery has no permit number. There is a plug on the load line. The tank battery is shared with permit numbers 2-3778, 2-3821, and 2-3764. The well was placed into production prior to House Bill 278. I called and spoke with KDA. I instructed KDA to repair the dike around the tank battery and posted a new identification sign with all of the permit numbers.
