Poulson Drilling Rig #1 continued to trip tubing out of the hole and laid down 52 joints (1500) that were full of cement (equivalent to 5.8 barrels of slurry). Tubing was run back in the hole and the bottom plug will be tagged tomorrow morning (11/25/15). The wellhead was packed in and operations were shut down. No violations.
0350850518 2015-11-20 Plug / Plug Back No Violations I was on location to continue with plugging operations. Appalachian Well Surveys was on location and ran a cement bond log to determine the top of the cement in the annulus between 7 production casing and 8.75 borehole. The log indicated the top of the cement at a depth of 7120. The rest of the hole was logged to find free pipe and formation tops. It was decided to have Appalachian perforate the Trenton Limestone at this time in preparation for the first of two squeeze plugs that would need to be set in the well. The 7 casing was perforated from 6946-6964 with 39 shots (0.62 entry). Appalachian rigged down and the wellhead was packed in for the weekend. No violations.
0546123872 2015-11-20 Plug / Plug Back No Violations Appalachian Well Surveys was on location to run a Bond Log to determine the top of cement in the annulus between the 7 and the 8.75 casing bore. The rest of the hole was logged to find free pipe. Appalachian also rigged up a gun and perforated the Trenton Sandstone formation with 39 shots from 6946 to 6964 for the perorations of the cement squeeze across the formation.

0575562167 2015-11-30 Plug / Plug Back No Violations I was on location to witness plugging operations. Poulson Drilling Rig #1 tagged the top of plug #4 (Rose Run Sandstone) at a depth of 7638 and is 170 into the 7 intermediate casing and meets requirements. Poulson pulled tubing to a depth of 6964 and Petroset Cementing rigged up to the pipe. They established circulation and pumped 30 barrels of freshwater and 100lbs of bentonite gel ahead of the slurry. They then mixed and pumped 30 barrels (135 sacks) of Class A cement and displaced the cement to 6464 with 25 barrels of saltwater. Poulson pulled 1000 of tubing to get above the cement and the wellhead and tubing were packed in. Petroset rigged up and the annulus between the 9.625 surface casing and the 7 casing was opened. Petroset squeezed 11.5 barrels of cement through the perforations at 1100psi-1400psi. They sat on the plug for 45 minutes and released the pressure. Less than 0.5 barrels flowed back to the truck. Poulson pulled tubing to 4954 and Appalachian Well Surveys arrived on location and built a 10 perforating gun with 20 slim hole shots (2 shots/ft.). They ran the gun down and out the bottom of the tubing and perforated the Medina Sandstone from 5000-4990. They came out of the hole and all shots had fired. Poulson spotted the tubing at 5005 and Petroset rigged up to the tubing. They established circulation and pumped 25 barrels of freshwater and 100lbs of gel ahead of the slurry. They then mixed and pumped 27 barrels (110 sacks) of cement slurry and displaced the slurry to 4620 with 18 barrels of water. Poulson pulled 900 of tubing, the well was packed in, and Petroset rigged up to the tubing. They squeezed 10 barrels of cement slurry through the perforations at 800psi and the tubing head began misting. They waited 10 minutes and released the pressure to the truck and 1.5 barrels flowed back. The tubing head was tightened and Petroset pumped the flow back (1.5 barrels) downhole and the plug was sat on for thirty minutes. The valve was released and no w
0581255474 2015-11-27 Plug / Plug Back No Violations Poulson Rig #1 ran tubing back in the hole and Apppalachian ran in with there wire line and tagged the plug ( plug 3 ) at 7916. 230 of the plug was lost to the formation. Poulson Rig #1 ran tubing to 7909 and Petroset rigged up to the tubing. They broke circulation pumped 30 barrels of fresh water followed by100lbs. Of bentonite gel and cottonseed hulls and shredded paper followed by 100 sacks of class A cement and displaced with 29 barrels of saltwater. Rig #1 pulled 25 joints packed off well head and shut until the week end.

0895005087 2015-12-03 Plug / Plug Back No Violations I was on location to witness plugging operations. Poulson Drilling Rig #1 spotted 4.6lb/ft., 2.375 tubing at a depth of 3200 and Petroset Cementing rigged up and began circulating the hole. There was a minor amount of gas and the hole was circulated and conditioned with 45 barrels of water. They then mixed and pumped 23 barrels (95 sacks) of Class A cement and displaced the cement to 2900 with 11 barrels of water to set the Big Lime plug. Poulson pulled tubing to a depth of 1127 and Petroset rigged up and began circulating the hole. They then mixed and pumped 16 barrels (65 sacks) of Class A cement and displaced the slurry to 927 with 3.5 barrels of freshwater to set the Berea Sandstone/Ohio Shale plug. Poulson pulled the tubing to 300 and Petroset rigged up, broke circulation, and then mixed and pumped 23 barrels (95 sacks) of Class A cement slurry. Cement was beginning to circulate up the 9.625 surface casing and we had to shut down. The trash pump for the circulating pit sucked up a rag and the pit was at capacity. Poulson pulled the last ten joints of tubing while they fixed the pump and got it working again. The pit was emptied to the frac tank, and the hose from the pump truck was placed in the 9.625 casing. The hole was topped off with 5 sacks of Class A cement until the desired consistency circulated. The cement level was holding 6 below the bottom flange of the wellhead an hour after topping the hole off. I will meet with Keith Moore tomorrow to verify that the top plug stayed up. No violations.
0901267233 2015-11-25 Plug / Plug Back No Violations Poulson Rig #1 tagged the top of the cement bottom plug ( plug #1 ) at a depth of 8660 ( 68 below the required depth ). One joint of tubing was pulled to 8645 and Petroset rigged up and bloke circulation and pumped 30 barrels of fresh water and mixed 100 lb. bentonite gel. They pumped 25 sack of class A cement and displaced with 33 barrels of salt water. The tubing was pulled to 7985 to set and squeeze the 300 plug across the Trempealeau and Rose Run Formations. Petroset rigged up and broke circulation and pumped 30 barrels of fresh water and 100 lbs. of bentonite gel followed by 80 sacks of class A cement and displaced with 30 barrels of salt water. There was 25 joint of tubing pulled. The well was packed off for the holiday.

1084349929 2011-10-19 Salt Water Injection Well No Violations The step rate test that was started on 10/18 was continued today with a frac truck and blender instead of an acid pump truck. The difference between the two pieces of equipment will allow for a higher injection rate. The test started at 1 barrel per minute (bpm) and was increased at 1 bpm increments every five minutes until 12 bpm at 1730 pounds per square inch (psi) was achieved without any apparent breakdown. Due to the volume of water used and still available, the decision was made to build pressure rather than rate. The test ended with a maximum rate of 16.5 bpm at 2,030 psi, again with no apparent formation breakdown.

Core Lab then placed radioactive ceramic beads (Scandium 46) in the top of the 7 casing and Superior Well Services displaced the beads using 430 barrels of brine at a rate of 5 bpm. This volume was calculated as hole/casing capacity plus 10 bbl. The maximum injection pressure was 1500 psi after full displacement. After the beads had been displaced Superior ran the gamma ray tool with the Spinner tool back to the bottom of the hole to quantify the bead distribution through the injection zone. (Refer to the log copies) The results indicated that the beads were concentrated in the upper open hole injection zone in the Rose Run, B Zone and Rome intervals. The beads were not present below 8730 in the Rome Formation.

Several attempts to run the Spinner were partially successful. The spinner ran when placed in the bottom of the 7 casing and when placed across the Rose Run. When placed any further down hole, the spinner stopped moving. The explanation was based on the relatively low injection rate (6 bpm) and the volume of fluid lost into the Rose Run sequence which both prevented sufficient fluid to pass through the tool.

They established circulation, and then mixed and pumped 30 barrels (125 sacks) of Class A cement slurry. The slurry was displaced to 8592 with 33 barrels of water at a circulating pressure of 1400psi. Poulson pulled 16 joints of tubing and Petroset attempted to flush/clear the tubing. The cement had u-tubed in the tubing and flash set. Poulson began tripping tubing out of the hole and shut down for the night. No violations.

1319719395 2011-10-18 Salt Water Injection Well No Violations Recent seismic activity in the Mahoning Valley has raised concerns that the deep (drilled to basement) salt water injection wells are facilitating these events. This well was drilled to a total depth (TD) of 9,038 with 7 casing set at 7,808 and 9-5/8 surface casing set at 1,027. The salt water injection zone extends from the bottom of the 7 casing to TD and includes the following formations, Rose Run, Copper Ridge, Conasauga, Rome, Mount Simon, granite wash and Precambrian basement. The purpose of todays actions were to determine formation breakdown pressure and where in this sequence the injected brine was going. On site personnel and contractors included Larry Smyers and Dave Jenkins (D&L Energy), Evan Zeller (Battelle), Larry Shamblin (Core Laboratories) and numerous Superior Well Service personnel providing open hole and cased hole logging tools, pump truck, crane truck and field hands.

Step I - Superior loaded the hole with 5.3 bbl of brine, tested lines to 5,200 psi and started the step test. The following table summarizes the step test:

1324681492 2015-12-09 Plug / Plug Back No Violations I was on location to assist Inspector Rick Skidmore in the plugging operation of the above well. J Well went in the 4.5 casing with a sand pump and tagged bottom at 4180. There was a good amount of fine sand in the bucket. It was tested by Inspector Bob Rousch and it was determined to be a Dolomite Lime Stone. They ran in with 1.5 tubing with a saw tooth cut collar and tried to drill but could not 1.5 tubing would only torque up. The decision was to set a bridge plug and a 200 cement plug and continue with the plug job.

1414911341 2015-11-19 Plug / Plug Back No Violations I was on location to witness plugging operations conducted by Moore Well Services & Poulson Drilling. A Baker Tools center spear was set in the 4.5, 11.6lb/ft. tubing. The Baker retrievable Hornet packer was unseated at 7707 and a 20 joint of tubing was pulled at a string weight of just over 90,000 pounds. The tubing was set in the rig slips just below the collar and the cut joint and center spear were laid down. Poulson recovered 7701 of 4.5 tubing and the 6 packer. The wellhead was packed in and operations were shut down for the evening. No violations.

1449763923 2015-11-16 Plug / Plug Back No Violations I meet with Keith Moore from Moore Well Service and Roy Poulson of Poulson Drilling to discuss the plugging of this well. The Rig is on location and also a 400 barrel 4 compartment flow back / water tank. Pressure was taken on the 4.5 tubing with my 3000 psi gage there was 325 psi the 7 had 280 psi and the annuls had 1100 psi. There was a 2 120 long flare line laid to the west of the location and the gas will be flared. The Niles Fire Department was notified and was on location an meet with Keith and myself, it was explained to them the fairing process.
1450186875 2015-11-19 Plug / Plug Back No Violations Poulson Drilling Rig #1 rigged up the 4.5 Baker Tool center spear and set the tool in the 11.6lb/ft. 4.5 casing and unseated the Baker retrievable Hornet packer. The first joint of casing was 20 in length before there was a collar, they set the casing in the rig slips and continuing pulling 4.5 casing.
1484354331 2015-12-04 Plug / Plug Back No Violations I was on location to check that the top plug in the well stayed up. I met with Keith and Jeff Moore (Moore Well Services) to check the top plug. There was water in the top of the hole and I found no indication that it was gassing. We used a field tape measure with a weight on the end to verify the top of the plug. The top of the plug was tagged at 42 below the top of the cut off casing and would need to be grouted to surface. Jeff bought a pallet of 94lb/sack, Type I Quick Crete to top the hole off. They connected 2-20 water hoses (2) together and ran one end to the bottom of the hole to displace (tremmie) the water. They poured 18 sacks into the 9.625 casing and worked the tremmie up and down and water flowed up the hose. The consistency of the cement at the top of the hole was acceptable. Keith tack-welded the ID plate on the surface casing and the well was buried. No violations.

1555631801 2015-11-18 Plug / Plug Back No Violations Attempts were made to remove the flanged well head. When the wellhead was removed the 4.5 was cut and no collar was available for them to connect there 4.5 elevators. Moores Well Service is gone to Baker Hughs yard to get a 4.5 center spear. The well was packed off until tomorrow 11/19/2015. There was also a 2 hour gas flare today.

Petroset rigged up to the tubing and boke circulation and pumped 30 barrels of fresh water and 100 lb of bentonite gel, followed by 135 sacks of class A cement The plug was displaced with 25 barrels of saltwater Rig 1 pulled 1000 of tubing above the cement and the tubing and wellhead was packed off and Petroset pumped 11.5 barrels through the perforations at 1100/1400 psi and held for 45 minutes. After pressure release there was .5 bbl flowed back the the tub.

1722037860 2015-11-30 Plug / Plug Back No Violations I was on location to witness Moores Well Service plug the injection well for D & L Energy. Moores contracted Poulson Drilling to plug the well. Petroset is the cement contractor. Petroset rigged up and established circulation. Petroset mixed and pumped 135 sacks of Class A cement. Next, the rig operator pulled 32 joints of 2.375 tubing and packed off the well. Petroset pressured up to 1,400 psi to squeeze the cement. Approximately 11 barrels of cement was squeezed for the Utica plug.

1907255510 2015-12-01 Plug / Plug Back No Violations I was on location to witness plugging operations. Poulson Drilling Rig #1 pulled the remaining tubing from the hole. They set a Baker Tools center spear in the 7 casing and attempted to pull stretch on the casing without success. They rigged up Appalachian Well Surveys, ran in the hole with a charge, and shot the 7 casing off at a depth of 3740. The casing jumped 3-5 indicating that it was free and parted. They unseated the casing hanger in the wellhead and pulled 25-30 of 7 casing to get to a collar and they set it in the rig slips. The top joint was laid down and the center spear was released. They rigged up the tongs and began pulling 7 casing. The casing was pulling about 20,000lbs over string weight due to the pipe being belled at the bottom where it was shot off. Poulson pulled 30 joints, packed in the wellhead, and shut down for the night. No violations.

They removed the top flange of the wellhead and found the 4.5 tubing has been cut off and there is no collar to grab with the elevators. They are currently shut down and Larry Poulson is picking up a 4.5 center spear from Baker Tools. Operations will continue tomorrow (11/19/15). The wellhead is packed in and everything is secure. No violations.
