0217383994 2013-02-04 Drill / Deepen / Reopen No Violations I witnessed the cementing of the conductor casing by Universal Well Services. Hole #1 had a diameter of 36 and a depth of 60. The Auger Rig reached hard sandstone at 40. The conductor casing was 30 and set to a depth of 60. Three 5 holes were placed in the conductor casing, 5 from bottom to allow for circulation. Universal broke circulation, pumped Type 1 cement, and displaced. There was a good return of cement to surface with 20 left inside the casing.
0248435538 2013-10-08 Production Wells No Violations I was asked by Deputy Chief Robert Warstall to go to the above location and observe the production operation of this well. I meet with Doug Oaks from Halcon and he explained to me the operation. During the week, nitrogen was used to asist the well to flow. During the operation a vapor from the nitrogen truck was released. This is a normal operation. This caused concerns from the neighbors. At present they are running a compressor to move the gas down the pipeline. When the compressor goes down it creates a large flair.
0532196931 2017-10-06 UrbanProduction Wells No Violations I found that Pin Oak Energy has taken ownership from Halcon on 9/12/2017. There is an identification sign posted at the well pad entrance. The well pad is secure with a locked gate. The well is in production. The flare is in operation.
0547723648 2013-04-26 Production Wells No Violations I was on location today due to a noise complaint called into the office. Halliburton was in the process of fracturing the well. They had been on location since 4/21/2013 and were on the 22 stage of 26 stages in the fracturing process. The supervisor for Halliburton informed me that they would be completed and off location by the evening. Halcon had a company come out while I was only location to take sound readings of various areas around the location. The readings taken at the edge of the property line closest to the nearest house was 85.5 decibel (dB). That was approximately 150 ft from the wellhead. The company man informed me that sound barriers will be utilized on future well site construction.

0700156652 2013-02-26 Drill / Deepen / Reopen No Violations I was on location to check on the status of the drilling operation. I signed in and checked in with the company man. The permit was posted in the company mans trailer. They were currently at 2829 feet and were drilling at about 6.5 feet per hour. The site was fenced. The locations drainage control was in place and in good repair. The spill containment was in good repair. The rig was on a spill containment liner and it was in good repair. While on site I obtained a copy of the blow out preventer test report. The test showed it passed. The blow out preventer was chained down. The air head was in place and not leaking. Fire extinguishers were in various locations around the site. There was casing pipe on location for the next stage of cementing. The pipe was 40lb, 9 5/8 inch pipe. The API number on the pipe was SCT-0381. The lease road was in good repair and constructed of lime stone.

I found during my inspection that the identification sign had been moved from the lease road entrance to the well pad entrance gate. I noticed that Halcon had not place the permit number on identification sign per my request. I called Mike McCardle and he stated a new sign was ordered; however it too did not have the permit number. Mike said the correct identification sign was received and waiting to be picked up from the Canterbury, PA office. Mike stated the new sign would be in place by 6/9/2014. I will follow-up on the requested action by 6/16/2014.

0806411421 2016-02-24 Production Wells No Violations I was on location for a status check. My inspection found the access road is in good repair. Identification signs are posted at the access road entrance. The pad is fenced and all man doors are unlocked. The #1V is in production. The well is equipped with a plunger lift assembly. There is a steel dike around the storage tanks.

The equipment on the pad consists of the following: 8 Storage tanks 1 Flare 2 Gas processing units (GPU) 2 Oil stabilization units 1 Dehydrator 1 Thermal electric generator (TEG) 1 Production well on the pad 1 Sand separator Site fenced and man doors locked


I called and spoke with Zane Oshurac from Halcon. I instructed Mr. Oshurac to secure all man doors with locks and post an identification sign at the well.

0855193829 2015-04-27 Production Wells No Violations I meet with the Federal EPA at this location. They performed an Air Quality Inspection of the well and tank battery. I performed a status check of the tank battery and well head. ID signs are posted. No violations were noted at this time.
0920700731 2013-09-30 UrbanProduction Wells No Violations My inspection found a sturdy cage built around the well for protection while drilling the next well. The cage was built with steel and had protective panels placed on all four sides. The cage was painted yellow with red reflective tape. The cage was properly secured to four rig anchors with chains. I took photos of the cage.

The barrier was anchored by I-beams; drilled 12 into the ground. The panels were made of a canvass with sound bending material. Each panel was approximately 15 long.

I found Neil Callahan, well tender for Halcon, working was on location. The well is equipped with a plunger lift assembly. Identification signs are posted at the access road entrance, on the well, and next to the tank battery. All of the man doors surrounding the pad are secure with chains and bolts and nuts. The access road is in good repair. The well pad is shared with permit number 2-4080.

1361476924 2013-02-20 Drill / Deepen / Reopen No Violations I witnessed Halliburton Well Service cement the 1st Intermediate Casing for Halcon. Patterson Rig 256 drilled Hole #3 to 1,535 with a diameter of 17.5. The casing size was 13.675 and set to 1,512. Halcon used a stab in method through the drill string. Halcon did not load the hole with fluid to circulate before pumping cement. Circulation was finally broke when Halliburton reached 156 barrels of cement. I observed a good show of fresh water, gel, and cement to surface.

1511951000 2013-10-07 UrbanProduction Wells No Violations I arrived at the Kibler well location at around 1:45 pm to look into the noise issue. The loud noise was from using nitrogen to kick off the well over the weekend. Also, nitrogen was released from Universal tanker trucks prior to injecting down the backside. The well was currently kicked off; however, Halcon is still dialing in their production facility which is west to southwest of the well. The flare was part of the production facility and being used to burn off flash gas from the production tanks and the returned nitrogen from kicking the well off. While on location, the flare did get loud for approximately 15 minutes. I asked Doug Oaks, Halcon production supervisor, about the loud flare. Doug stated the compressor went down and they needed to vent the gas until the compressor was restarted. I took to videos of the flare while it was burning off flash gas. The first location of the video was from within 30 ft. and the other was from the tree line east the flare and production equipment.
1614231490 2018-01-04 Production Wells No Violations I conducted an inspection of this well site with DOGRM inspector, John Fleming. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. Production records for this welll were current. The tank battery consisted of eight 400 BBL production tanks and was diked. Other production equipment on site included two mechanical separators, one line heater, and one GPU. The site is shared with well 2-3H (2-4080). The secondary eathern berm was intact and the pad was stable. The signage was located at the main locked gate. The lease access was stable. No violations observed.
1634935271 2013-06-19 Production Wells No Violations My inspection found Halcon flaring the well without any prior notification. The flaring operations began on 6/18/2013 at approximately 3:00 am. I informed Mike McCardle from Halcon that I wanted notified 24 hours in advance for any future Halcon flaring operations. The flare had the correct spacing from the wellhead. The entire site was fenced with a locked gate. Flaring will be for approximately 2 to 3 weeks. Halcon did notify the Lordstown Fire Department 24 hours prior to beginning operations.
1840116135 2014-05-06 UrbanPreliminary Restoration No Violations I was on location to perform a urban preliminary restoration inspection. I found during my inspection the well was in production. The tank battery consisted of eight, 400 barrel production tanks, a compressor, and separators. The production equipment was shared with permit number 2-4080. A steel and lined dike surrounded the tanks. A flare was burning off flash gas from the production tanks. An identification sign was posted at the entrance to the lease road. Warning and safety signs were properly posted. Safety equipment was installed on the tanks. The lease road was in good repair and shared with two NCL wells. There was a locked gate at the entrance to the well pad. All drainage and erosion controls were in place. The well and production equipment were surrounded by a wooden post and wire fence. The entire area around the well pad was being used for agricultural purposes by the landowner.
1849113114 2013-09-03 Drill / Deepen / Reopen No Violations My inspection found Halcon building the pad for an above ground water impoundment and installing the steel dike for the tank battery. A 20 high sound barrier was installed on the entire east side of the well pad. I noticed no violations at this time.
1850412355 2013-07-31 Production Wells No Violations I inspected the shut-in well with ODNR Tom Hill. We notice a gurgling sound coming from the well. I called Mike McCardle, Halcon supervisor, and told him to dispatch the proper crew to investigate the issue.
