0403338040 2016-10-19 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. During my inspection the well is in production equipped with a production tree well head. A steel line with a gauge located at the base of the well head that is reading the 13.375 inch diameter casing annular pressure is slightly leaking causing the rain water to bubble around the line. I called Tony Long with PDC Energy and explained to him my findings. He stated that he will be sending someone out to check on the situation.

0508416569 2014-01-06 Drill / Deepen / Reopen No Violations I was notified by Mr. Tony Long by e-mail that there had been a small oil-based mud spill on the Garvin Pad during the process of moving the rig off of the location. He had also sent me pictures. There was less than 5 gal and it was contained within the pad. When I observed the site, everything had been cleaned up and no evidence of the oil-based drilling mud was left.
0654858472 2013-10-27 Drill / Deepen / Reopen No Violations Witnessed the cementing of the surface casing. 450 ft. of 24 in. hole was drilled and 430 ft. of 20 casing was ran. I tested the mix water to be used for the cement. It showed no chlorides. Cementing company had trouble getting the cement head to seal. The decision was made to weld the head to the casing. Good circulation and good returns were observed at the surface.

1316950686 2017-11-27 Production Wells No Violations The inspection this day found that the well is producing using a plunger lift system. The valves are open on the wellhead and gas could be heard flowing through the well at the time of inspection. The tank battery for this well is equipped with six (6) 410 barrel tanks with identification posted near the tank battery. There are no issues to report and the well is in production.
1387574243 2013-12-20 Drill / Deepen / Reopen No Violations Total depth of the well at the time of inspection was 15,531 ft. The hole was being conditioned and crew was getting ready to run 5.5 in casing. Spoke with company man on site.
1421389163 2013-09-30 Drill / Deepen / Reopen No Violations Witnessed the running and cementing of the conductor pipe. 100 ft of 30 in pipe was ran and cemented to surface. Good circulation and good returns were observed at the surface.
1435001228 2015-06-22 Production Wells No Violations Upon inspection, it was observed that this well is currently producing. The well is equipped with a standard production tree and is producing on the 5.5 production casing as well as the 2.875 tubing. Gauges on the well read 471 psi on the tubing and 500 psi on the casing. There is a small leak on the gauging nipple on the 13.375 intermediate casing and gas is bubbling up around it in the cellar. I contacted Mr. Tony Long of PDC and informed him of the leak and he stated their production supervisor would take a look right away and fix the issue. The tank battery consists of 6 400 bbl production tanks and 3 large mechanical separators. There is also 3 flow control separators on location as well as a large degassing tower. There is a metal, poly lined containment dike around the tank battery. Legible and correct ID information is present on the tanks as well as the entry to the lease road. No violations were observed.
1624903103 2014-07-14 Preliminary Restoration No Violations Upon inspection, it was observed that a vegetative cover has been established throughout the limits of disturbance for this pad location. Any and all drilling equipment has been removed from the location, and any and all pits used for drilling activities have been filled. This satisfies the requirements for the preliminary restoration. Accompanied on this inspection by DOGRM Engineer Jason Simmerman, and DOGRM Engineering Technicians Andy Thomas and Anthony Carson.
