2070050691 2016-04-04 Production Wells No Violations Inspected tank battery on 04/04/2016. The tank battery consists of 1-100 bbl steel tank, 1-1,100 gal plastic tank, and 1-vertical separator located within an earthen dike. Well identification is clearly displayed at the tank battery. The lease road to the wellhead crosses a stream that was too high to cross either on foot or in a truck due to recent rain. Will attempt to inspect the well at a later date. 2139391620 2018-03-05 Field Inspected, Well Not Found No Violations Inspected location on 03/05/2018 at the request of Mark Sparr (James R. Smail, Inc.) to confirm the status of the well. No surface expression of the well was noted. Research into the history of the well showed that it was plugged from the drilling rig in 1987 due to a lost hole. Production data and inspection reports for the Shelly #2-A well (PN: 169-24487) have been incorrectly attributed to this well. This was brought to the attention of Mr. Sparr and Gregg Miller (DOGRM Columbus) so that the well status can be changed and production and reports attributed to the correct well. No other problems were noted at the time of the inspection.
