2375741 Primary Facility 2015-06-03 Compliance Evaluation No Violations Noted On 4/29/15, I performed a follow up inspection on the Norris, David et al site reviewing the cleanup of the frac fluid spill from 2/23/15. Visual inspection of the site indicated that the site appeared to be in compliance with department policy and that, with the exception of backfilling the excavated area of the pad (pending incident closure), the site has been reclaimed.

2659575 Primary Facility 2017-11-13 Routine/Complete Inspection No Violations Noted Post-Drilling well Site Restoration Report received on 9/22/2017. At the time of inspection the well was drilled and equipped for production. Well apparatus, gas production units, and brine tanks appeared to be functioning. Earth disturbance authorized by the permit appear to be complete. No observation of accelerated erosion or sedimentation in relation to the well pad or access road.
