2684852 Primary Facility 2018-01-22 Routine/Complete Inspection No Violations Noted At time of inspection, well drilled and appears equipped for production. No brine tank on-site.

The Department observed a moderate gas odor and a hissing noise coming from the well. There are several structures located within an approximate distance of 200 feet from the well. These observations were forwarded to the OGI for follow-up as appropriate.

2684954 Primary Facility 2018-01-22 Routine/Complete Inspection No Violations Noted This well is tagged and shut in waiting on the Operator to plug it. The well is on a consent order to be plugged and an NOI has been issued. I responded to inspect this well at the request of a water quality specialist who reported that this well was leaking gas. The well is leaking gas from the 8" casing annular space and I contacted the operator to request that the annular space be vented10' above ground level into the atmosphere to more safely disperse the small amount of venting annular gas until the well is plugged shortly. The operator will have this venting request done by later this week. I measured the air going away from the well and while I could smell some gas up to 40' away from the well, my meter did not register any gas at that distance or any farther away from the well.
2729286 Primary Facility 2018-05-14 Plugging(Includes Plugged/Mined Through) No Violations Noted The plugging Rig has just started plugging operations on this well. The well was opened up at 40 psi, blown down and the present TD is 1615'. The original TD was 3273'. There is open hole through the 3" tubing down to 1369'. The Rig was waiting on water to arrive so they could attempt to circulate down at the time of this inspection. Matting was put down from the access road back to the well. All equipment on site looked ok.
2730672 Primary Facility 2018-05-16 Plugging(Includes Plugged/Mined Through) No Violations Noted The plugging rig set the bottom cement plug, set down on cement at 1158' and was in the process of shooting off the 3" and pulling it to set the next cement plug before logging the 6" pipe at the time of this inspection. All equipment on site looked ok.
2731037 Primary Facility 2018-05-17 Plugging(Includes Plugged/Mined Through) No Violations Noted The plugging rig set the bottom cement plug, set down on cement at 1158' and shot off and pulled the 3" production casing. The 6" casing was badly eaten away and not even present at the surface. The crew was on the process of perforating the 6" from the bottom of the string at 1173' with ten shots every 100' up to 200' at the time of this inspection and then plans on squeezing cement in the morning. All equipment on site looked ok.
2823763 Primary Facility 2019-01-08 Site Restoration No Violations Noted Inspection conducted in response to a Well Site Restoration Report received by Department.

At time of inspection well is plugged. Monument present with permit number permanently attached. No temporary BMP's on site. Access road and site appear returned to near original contours. Site vegetated and appears to have reached >70% uniform growth. This Restoration Report is approved.
