2697039 Primary Facility 2018-02-22 Follow-up Inspection Violation(s) & Outstanding Violations This well is not tagged and is in production. The 2" production tubing is severely corroded at ground level and is compromised resulting in the leaking of steady gas to the atmosphere which has the potential to pose a hazard to the public. The operator must either repair this well and restore the mechanical integrity of the well or else plug the well according to the requirements of the the Oil & Gas regulations. In addition, the operator did fail to submit both the required annual well production report and the required annual mechanical integrity report for this well.
2702269 Primary Facility 2018-03-06 Follow-up Inspection No Violations Noted On this date the Department's emergency contractor cut the 6 5/8" casing off to a point 3' below ground level to where it was good and then cut and threaded the 2" production tubing from 18" below grade and plumbed a new 2" production tubing riser 4' above grade and installed a 2" valve with a gauge and shut the well in with the gauge on to monitor back pressure on the 2" production tubing. The contractor then installed a new piece of 8" casing over the old 6 5/8" well casing to above grade and backfilled around it for stability. The well rocked up to 20psi within one hour of being shit in. I checked the outside of the 2" well production casing and the area between the 2" well production casing and the 6 5/8" casing for the presence of gas and there was no gas present at this time.
2702280 Primary Facility 2018-03-07 Compliance Evaluation No Violations Noted I returned to monitor the status of this well on this date and noted that since being shut in the day before after the emergency contract work was performed on this well it has now rocked up to 52psi in 24 hours since it was shut in. When I placed the sampling nozzle of my Altair 5X meter into the open space between the shut in 2" well production tubing and the 6 5/8" well casing my meter detected the presence of 2% methane escaping from between the well casings. This reading was not sustained however and it came and went from non-detect to 2% back and forth during the 5 minute time period which I monitored it. There did not appear to be any measurable volume of gas escaping from the surface of the well casing into the atmosphere around the well as when I sampled from 2 feet away from the well 360 degrees around the well for approximately 5 minutes, my Altair 5X meter remained at non-detect the entire time on this outside perimeter of the well. 2702305 Primary Facility 2018-02-28 Routine/Partial Inspection No Viols - Permanently Shut Down This abandoned well is leaking gas from the 2 3/8" tubing at ground level. Production casing is open and filled with debris and rainwater, causing tubing to rust badly. Emergency contract to remediate the leak will be pursued.

2702351 Primary Facility 2018-03-06 Routine/Partial Inspection No Viols - Permanently Shut Down Allegheny Well Service was selected for remediation of leak in the 2 3/8" tubing. Work began before 9:00 AM and was successfully completed by noon. After excavating around wellhead, surface and production casings were cut off to expose the damaged 2 3/8" tubing. The bad section of tubing was cut off, threaded and replaced by a new section of tubing, topped with a Balon valve and tee with a tapped plug and 400# gauge on one side and a solid plug on the other. Well pressured up to 20# after 1 hour SI.
2702548 Primary Facility 2018-03-08 Compliance Evaluation No Violations Noted On this date I conducted another post emergency contract repair inspection on this well. The shut in pressure on the well has not changed in the past 24 hours and remains at 52 psi. My Altair 5X meter with its short probe did not detect any gas coming from the annular space between the 2" production tubing and the 6 5/8" well casing. So I utilized my 62S Gas Scope meter with the 36" probe and placed the probe down inside the 8" surface casing and monitored the well casing for approximately 5 minutes. The meter was at non-detect most of this time and only registered a brief reading of 35% LEL for about two seconds and then this slight show of gas disappeared and did not return. I sampled the atmosphere in the immediate area all around the well for approximately 5 minutes and both my Altair 5X meter and my 62S Gas Scope meter remained at non-detect for the presence of gas. This well appears to be stabilizing at this point. I will return to monitor and document it again tomorrow and on next Monday as well.
