At time of inspection, well does not appear to be plugged. Temporary BMP's installed per the E and S plan.

2780554 Primary Facility 2018-09-27 Plugging(Includes Plugged/Mined Through) No Violations Noted The Rig crew has had this well open for most of two days now attempting to get the well to blow down. They opened it up with 510 psi on the 2" and 150 psi on the 8" and the well was still blowing hard, steady gas with no fluid pulsation evident while I was on location. I asked them how they plan to safely kill this well to plug it and they said the operator will be getting a pump truck out to bull head pump down and kill the well shortly if they can't get it killed. I contacted the Operator's plugging Supervisor to verify this and requested that they be careful with this well. All equipment on site looked ok.
2782248 Primary Facility 2018-09-27 Administrative/File Review No Violations Noted Operator rep Jeff Osburn contacted Department by phone. Mr. Osburn stated rain water is running across access road at this time due to recent rain fall. Mr. Osburn requested Department permission to install a culvert pipe to control the rain water. Department recommends operator delay any BMP installation until a site inspection can be conducted. Department intends to visit site tomorrow morning.
2782287 Primary Facility 2018-09-28 Routine/Partial Inspection No Violations Noted Routine partial Inspection conducted in response to operator requested guidance on temporary BMP installation.

Operator rep Mike Gatskie present during inspection. E and S plan on site. At time of inspection contractors are placing rock on access road. Mr. Gatskie states water from a recent rain fall was running across access road. Mr. Gatskie instructed contractors to add rock to access road to slow run off. At this time a small amount of water is crossing the access road near the well pad. This water appears clear, not sediment laden. This area of the access road is rocked (photo #1) but, according to Mr. Gatskie more rock will be added today. On a 9-27-18 phone call with Department, operator rep Jeff Osburn inquired about installing a culvert pipe across access road to control rain water (see inspection record #2782248). Department does not recommend a culvert pipe to address this rain water issue. It appears the rock added to the access road is filtering and slowing the rain water sufficiently. The
