2099879 Primary Facility 2012-09-14 Incident- Response to Accident or Event In Compliance with Policy Well located in a cultivated crop field. Prior to the compromising of the production fluid tank the well had been equipped for production. Production fluid tank had been compromised by what appeared to be a small hole as a result of a firearm. As a result of the compromised tank crude oil which had accumulated on top of the production fluids had discharged to the diked area around the tank. The discharge was discovered and reported on 09/13/12. At that time the tank was disconnected and the operator began contaminated soil excavation. At the time of the inspection the operator had begun to backfill the dike and reconnect the well. Soil samples had been taken from the excavated area in addition to background samples. A full report on site remediation from the operator would be forthcoming to the attention of WQS Greg Higgins.
2110314 Primary Facility 2012-10-24 Routine/Complete Inspection No Violations Noted Well located in a cultivated crop field. Site excavation had been completed and a review of the site remediation report and sample results show that the contaminated media had been removed. Excavated soil onsite covered by and on top of plastic. The reconstructed berm had been seeded and mulched and the vegetation had begun to take hold.
