2368628 Primary Facility 2015-05-08 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and it appeared to be in sound condition. It was noted that the access road required routine maintenance of the access road erosion controls. Specifically what appeared to have been water bars and roadside ditches required regrading to prevent runoff from flowing down the road. Additionally a collection ditch implemented at the bottom of the cut slope for the well pad required maintenance as well to minimize the erosion and sedimenation generated from runoff leaving the site.
2382763 Primary Facility 2015-06-25 Routine/Complete Inspection Violation(s) Noted Well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and it appeared to be in sound condition. Due to recent precipitation events and inaction by the operator after receipt of the 05/08/15 inspection report for this site that outlined the necessity of work to repair and maintain access road erosion controls the erosion controls have further deterioated. The erosion ditches that had begun to form are now significantly deeper and more sediment has been generated and transported off the road. At this time the neglect of the access road controls has resulted in a violations of PA Code 25, Chapter 102.
2398987 Primary Facility 2015-08-17 Follow-up Inspection Outstanding Violations - No Viols Req'd Well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and it appeared to be in sound condition. Operator had taken initial action to begin addressing the E&S issues previously outlined with the access road. The road had been regraded and the ditches and ruts had been removed. At this time the operator still needs to implement permanent erosion controls, i.e. ditches, culverts, outlets and/or rock stabilization to fully address the previously outlined violations and achieve compliance.

WQS Higgins met onsite with operator and contractor representatives to review the necessary permanent erosion controls for the access road.
