1776383 Primary Facility 2009-03-13 Routine/Complete Inspection Violation(s) Noted According to Oil & Gas Inspector documentation drilling began on 04/07/08. At the time of the inspection the well appeared to be equipped for production. 1) Noted that the pit liner had fallen down at one end of the pit and the liner had been torn on the side closest to the cut wall on the well pad. Noted very little fluid in the pit, however it was in contact with the exposed earth due to the fallen liner. 2) Noted that the site had not been regraded, seeded, or mulched. 3) Found no well tag on the well apparatus located onsite.
1785654 Primary Facility 2009-04-21 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Below grade pit that had been located onsite had been filled in, and the site had been regraded. Permit number noted on the well apparatus. As per phone conversation with the operator the well is to be stimulated in the near future. Drilling was most likely completed in April of 2008, and thus the 9 month restoration time window would have expired in 02/09, therefore the well would be in violation of the site restoration regulations set forth in 601.206c. Specifically the site does not meet the 70% uniform vegetative cover criteria, nor is another permanent BMP implemented capable of resisting accelerated erosion & sedimentation. Additionally the operator has not applied for an extension to the restoration time frame as outlined in 601.206g. At this time the Department is requesting that by 05/15/09, the operator forward a preliminary copy of the well-record and completion report showing the date on which drilling was completed.
