1757892 Primary Facility 2008-11-19 Routine/Complete Inspection No Violations Noted Met onsite with Wes Nelson of Fortress Energy to review the E&S plan. Found the following deficiencies with the E&S plan drawing: 1) The operator should show exactly what BMP's will be utilized on the access road (i.e. energy dissipators at culvert inlets and outlets) to minimize accelerated erosion & sedimentation. 2) The filter fabric fence should be straightened and should follow the contour of the site. As it is shown on the plan drawing, it would not be effective in minimizing accelerated erosion & sedimentation. In addition to minimize the disturbed area the operator could move the soil stockpile so that it is above and adjacent to the well-site
1929388 Primary Facility 2010-11-22 Site Restoration No Violations Noted Well is equipped for production. Well tag located. Brine Tank appears to be in sound condition. There is no evidence of erosion or sedimentation on the access road or well pad. The vegetation is well established. At this time, the site is restored.
