1805894 Primary Facility 2009-05-28 Routine/Complete Inspection No Violations Noted Well staked site not constructed at the time of the inspection. E&S plan as outlined should minimize accelerated erosion & sedimentation provided the plan is implented & maintained properly. Noted that the seasonal high groundwater table was at a depth of between 17 and 22 inches. In addition due to the shallow depth to the seasonal high groundwater table encapsulation of the drill cuttings is not available as an option to dispose of said cuttings. Also land application for disposal of cuttings is available only if the if the seasonal high groundwater table can be shown to be at least 20 inches from the surface. At this time the Department is requesting that the operator revise and submit the E&S plan and waiver application to show a method of disposal for the drill cuttings by the date given on the accompanying letter.
