Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1865803 Primary Facility 2010-02-25 Routine/Complete Inspection No Violations Noted Drill rig was located onsite at the time of the inspection. Two below grade pits were observed onsite. Both pits had more than the required two feet of lined freeboard. Observed no accelerated erosion & sedimentation with respect tot eh access road or well-site at the time of the inspection. Silt fence had been trenched in properly. Noted that the silt fence had not been implemented on contour below the well-site facing the UNT to Campbelll Run to the west of the well. The installation of silt fence in this way could result in the accumualtion of runoff to a single low point along the silt fence and thus compromise the fencing. In this instance a collection ditch with sediment trap may have been a more functional erosion control.

1871257 Primary Facility 2010-03-03 Drilling/Alteration No Violations Noted MARCELLUS - tophole completed to 1150ft - awaiting downhole rig/E&S ok/ID&pit ok
1872821 Primary Facility 2010-03-16 Drilling/Alteration No Violations Noted Patterson - UTI moving equipment on location
1872823 Primary Facility 2010-03-25 Drilling/Alteration No Violations Noted Contractor @ 1751' & dusting; Pit, E&S OK
1878109 Primary Facility 2010-04-20 Routine/Complete Inspection No Violations Noted Drill rig was in the process of being moved off-site. Below grade pit located onsite and it contained a minimal amount of fluids along with the drill cuttings. The pit had more than the required two feet of lined freeboard. Frac pits had been constructed, however they had not been lined. All erosion control BMP¿s appeared to have been installed correctly, and no accelerated erosion & sedimentation was observed in association with the access road or well-site. The pits and erosion control BMP¿s should continue to be monitored until well development is completed. The operator should also undertake site restoration activity as soon as possible after the completion of well development activities.
1878684 Primary Facility 2010-04-14 Drilling/Alteration No Violations Noted Drilling complete @ 12,100ft - cleaning hole/E&S ok/ID&pit ok
1878699 Primary Facility 2010-04-06 Drilling/Alteration No Violations Noted MARCELLUS - drilling horzontal @ 7200ft/E&S ok/ID&pit ok
1884096 Primary Facility 2010-05-13 Routine/Complete Inspection No Violations Noted Inspection performed as a follow-up to a report of a bentonite spill.
1888016 Primary Facility 2010-05-28 Routine/Complete Inspection No Violations Noted Site inspection performed in response to stream crossing and bentonite mud concerns expressed by the operator. Met onsite with EXCO reps Perry Lupinetti and Russ Lichty. Area of concern is the stream crossing of the pipeline on sheet 5 of the ESCGP-1 covering this project (ESX10-005-0001). All E&S controls outlined in the ESCGP-1 had been implemented properly, and the operator had implemented additional measures to prevent any bentonite mud from reaching the unnamed tributary (UNT) to Campbell Run. These additional measures appeared to be adequate to prevent any mud from entering the stream. No adverse effects were observed in the UNT. The operator should continue to monitor this site to insure that all e&s controls continue to function properly and to insure that any bentonite mud does not enter the UNT.
1958038 Primary Facility 2011-03-16 Routine/Complete Inspection No Violations Noted Drill rig had just been moved to the Lenape Heights from the Iseman well pad. R. Iseman 3H was tagged properly and equipped with a production unit, however due to drilling which had been ongoing for other wells on the well pad the well was not in production at the time of the inspection. Brine tanks located onsite and appeared to be in sound condition. Noted that there was a metal containment system in place around the tanks. Observed no accelerated erosion sedimentation in relaition to the site or access road. Specifically there was evidence of sediment transported off-site while moving the drill rig and ancillary equipment to the Lenape Heights well pad.
2042778 Primary Facility 2012-02-15 Routine/Complete Inspection No Violations Noted Drilling had been completed, well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Erosion controls had been implemented and maintained as outlined in the approved ESCGP.
