1925072 Primary Facility 2010-11-02 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite. Observed that a rock had torn the liner near the top. Instructed drill crew that the fluid level in the pit must remain at least two feet below the compromised area of the liner. Also observed what appeared to be some rocks that were putting pressure on the pit liner. Recommended to the drill crew that when possible to have some of the fluids removed so as to relieve pressure on the liner and reduce the risk of the liner being compromised further. Crew member onsite stated that the pit had been liner with a felt liner prior to lining the pit with plastic. Noted that the silt fencing had not been implemented, but a collection berm had been created at the bottom of the fill slope. Noted that the access road had minimal erosion controls, as outlined in the E&S plan. No accelerated erosion & sedimentation was observed on either the access road or well-site at the time of the inspection.
1928103 Primary Facility 2010-11-17 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Drill pit still located onsite and two holes in the pit liner were observed. These holes were more than two feet above the fluid line and thus the pit has adequate freeboard. However due to the compromised liner the cuttings will not be able to be encapsulated as per the E&S plan. Also due to the compromised liner and the obvious rocks seen under the liner causing additional pressure points the Department is requesting that the cuttings be land-farmed or hauled to a landfill and the drill pit regraded as soon as possible. The Department is requesting that the operator forward documentation or contact WQS Higgins via phone at the number listed at the top of this inspection no later than 11/24/10 with the operator¿s choice to address the drill pit issue.

1948014 Primary Facility 2011-02-07 Routine/Complete Inspection No Violations Noted Drilling had been completed. Well appeared to be equipped for production. No brine tank located onsite. Below grade drill pit still located onsite. Noted that one end of the pit liner had fallen down into the pit. Observed minimal fluids and snow in the pit. The Department recommends that the operator regrade the drill pit as soon as possible so as to minimize the possibility of precipitation and snow melt building up in the pit and the possibility of fluids from the pit being absorbed in the exposed earth. The operator should endeavor to address the pit liner issue as soon as the weather permits equipment access to the site. Observed a bulldozer and trackhoe at the base of the access road which may be utilized to reclaim the drill pit and land farm the drill cuttings.
