Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1885019 Primary Facility 2010-05-14 Routine/Complete Inspection Violation(s) Noted Crews were on-site at the time of the inspection. Site ID and E&S plan located.
1887956 Primary Facility 2010-05-13 Drilling/Alteration No Violations Noted drilling/E&S inadequate - notified WQS/ID&pit ok
1887959 Primary Facility 2010-05-13 Drilling/Alteration No Violations Noted drilling/E&S being evaluated by WQS/ID&pit ok
1887961 Primary Facility 2010-05-14 Drilling/Alteration No Violations Noted drilling/E&S ok/ID&pit ok
1893445 Primary Facility 2010-06-02 Compliance Evaluation No Violations Noted CEI performed as a follow-up to a previously issued NOV.
2008613 Primary Facility 2011-10-06 Routine/Complete Inspection No Violations Noted Well operating and properly identified.
2009567 Primary Facility 2011-10-06 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Access road and site erosion controls had been implemented as outlined in the E&S plan. There was no observed accelerated erosion or sedimentation in relation to the site or access road. A large frac pit was noted onsite located below the well pad. The pit appeared to contain flowback fluids and it had more than the required two feet of lined freeboard. The Department recommends that the operator endeavor to begin site reclamation activities as soon as possible or implement temporary erosion control measures that will insure minimization of accelerated erosion and sedimentation through the winter until the next growing season.
2039803 Primary Facility 2012-02-03 Routine/Complete Inspection Violation(s) Noted Drilling had been completed. Well appeared to be equipped for production at the time of the inspection. Brine tanks located onsite and they appeared to be in sound condition. Site had been regraded, however it had not been seeded & mulched or otherwise temporarily stabilized. In addition the access road as it crosses a small unamed drainage way does not have adequate erosion controls to minimize accelerated erosion or sedimentation. Specifically the dirt access road did not have any culverts, broad based dips or other erosion controls to break up total runoff, divert runoff off the access road, or othersie minimize sedement generation or transport. It was noted that the current access road enters the well area from a different direction than the road during well drilling and stimulation. Spoke with operator reprsentative Tyler Krukar via phone regarding the well site and the lack of access road erosion controls.
2044791 Primary Facility 2012-02-23 Follow-up Inspection Outstanding Violations - No Viols Req'd Drilling had been completed. Well appeared to be equipped for production at the time of the inspection. Brine tanks located onsite and they appeared to be in sound condition. Site had been regraded, however it had not been seeded & mulched or otherwise temporarily stabilized. The operator had begun to implement the access road erosion controls, specifically the culverts had clean durable rock placed at the inlets and outlets, and a rock lined turn out had been implemented on the hill side to convey runoff off of the access road.
2054523 Primary Facility 2012-03-28 Compliance Evaluation No Violations Noted Drilling had been completed. Well appeared to be equipped for production at the time of the inspection. Brine tanks located onsite and they appeared to be in sound condition. Site had been temporarily stabilized with mulch and it was noted vegetation had begun to grow over the majority of the well pad. The operator had also completed access road erosion control upgrades. The operator should continue to monitor the site for erosion issues and to insure that the 70% uniform coverage criteria is met.
2081138 Primary Facility 2012-07-05 Site Restoration No Violations Noted At the time of inspection the well was drilled and equipped for production. The well apparatus and brine tank located onsite appear to be in sound condition. The Department also notes that the site is restored and meets the 70% uniform vegetative coverage criteria.
2366619 Primary Facility 2015-05-01 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimenation in relation to the access road.
2623703 Primary Facility 2017-08-09 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the access road.
2798101 Primary Facility 2018-11-05 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The annulus is open. The well head casing, piping, production set equipment and two grounded 100 bbl fiberglass fluid collection tanks look ok.
