1918565 Primary Facility 2010-10-07 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit located onsite and it had more than the required two feet of lined freeboard. Site erosion controls had been implemented as outlined in the E&S plan. Noted in the E&S plan there were inadequate erosion controls. Specifically the plan did not utilize an effective method to deal with runoff on the access road and the short length of road and proximity to the paved road necessitated the rocking of the entire access road. Contacted operator representative Robert Carnahan regarding this matter. Mr. Carnahan called WQS Higgins back and stated that the road will be regraded to inslope to the cut side, a collection ditch rocked with rip-rap for stability will be implemented in the next few days. These actions should sufficiently address the access road concerns noted by WQS Higgins.

Brine tank located onsite and appeared to be in sound condition. Site had been regraded. Observed no erosion or sedimentation issues in relation to the access road or well-site. The operator should insure that the vegetative cover take hold and achieves the 70% uniform coverage criteria. In addition the operator should have the well tagged with the appropriate permit number as soon as possible.
