1998220 Primary Facility 2011-08-24 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed. Drill pit located onsite and it contained fluids and drill cuttings. Pit had more than the required two feet of lined freeboard. Observed no accelerated erosion or sedimentation in relation to the access road or well-site. The operator should continue to monitor the erosion controls to insure their continued functionality until well development activities are completed.
2047745 Primary Facility 2012-03-05 Routine/Complete Inspection No Violations Noted Drilling was completed in June 2011. Well was not equipped for production. Drill pit onsite and it contained fluids and drill cuttings. Pit had more than the required two feet of lined freeboard. Site erosion controls were in place and there was no evidence of accelerated erosion or sedimentation. Noted that the well was approaching the end of the 9-month restoration window. At this time the Department is requesting that the operator forward documentation outlining an anticipated date for completion of well development and site restoration acitvities. This documentation should be forwarded no later than March 22, 2012.
2104452 Primary Facility 2012-10-02 Site Restoration No Violations Noted At the time of inspection the well was drilled and equipped for production. The associated brine tanks and well apparatus appear to be in sound condition. The well pad and access road appear to be in sound condition with no observation of accelerated erosion or sedimentation. The well pad areas appears to meet the 70% uniform vegetative criteria need for restoration. The site is restored and the Department suggest the Operator remove any temporary perimeter E&S controls that may still be located onsite. The Department notes that at the time of inspection the Department could not read the well tag. The Department suggest the Operator install a legible well tag as soon as possible. The Department also notes what appears to be an unrestored rock borrow pit located just off the well pad and access road at the following coordinates 40 55 22.9N, 79 35 12.6W. The Department suggest the Operator regrade, seed and mulch the rock borrow pit as soon as possible.
2170523 Primary Facility 2013-05-29 Site Restoration No Violations Noted Well appeared to be equipped for production, brine tank located onsite. Brine tank appeared to be in sound condition. Vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation observed in relation to the site or access road.
