2013688 Primary Facility 2011-10-26 Routine/Complete Inspection Violation(s) Noted Site construction had been completed, drilling had not commenced at the time of the inspection. E&S controls had been implemeneted as outlined in the E&S plan. Access road controls should effectively minimize accelerated erosion and sedimentation as it relates to the access road. Noted that the access road is shared with two residences near the well pad. The site erosion controls implemented where inadequate to eefectively minimize accelerated erosion & sedimentation. Specifically silt fencing and hay bales had been installed where collection ditches from either side of the well pad converge and result in a concentrated flow of runoff. It is apparent that prior to site construction that this is the direction that runoff flowed.
2017543 Primary Facility 2011-10-27 Drilling/Alteration No Violations Noted Operator provided spud notification to begin on 10-27-11. No equipment on site at time of inspection. Pit lined and freeboard maintained.
2017941 Primary Facility 2011-11-09 Follow-up Inspection Outstanding Violations - No Viols Req'd Drilling appeared to have been completed. Equipment was in the process of being set up for well development. It was noted that a small sediment trap had been installed in the area requested as a result of the previous inspection on 10/26/11. This sediment trap should have been larger and the approaching collection ditches should have been stabilized with either clean durable rock or erosion control matting of some nature and seeding to minimize the generation and transport of sediment.

Brine tanks located onsite and they appeared to be in sound condition. The site appeared to have been recently regraded and the access road and ditches finished with clean durable rock. The operator should seed & mulch the site as soon as possible to insure complaince with 102.22(b). There was no observed accelerated erosion or sedimentation in relation to the access road or well-site.
2070004 Primary Facility 2012-05-22 Site Restoration No Violations Noted At the time of inspection the well appeared to be drilled and equipped for production. The well apparatus and brine tanks located onsite appear to be in sound working condition. The site had recently been seeded and mulch but does not meet the minimum vegetative standard at the time of inspection. The Department recommends that the Operator continue to monitor the site to ensure 70% uniform vegetative coverage criteria is met. No accelerated erosion and sedimentation observed is relation to the well pad area or the access road.
