Site construction and E&S controls had been implemented as outlined in the approved E&S plan. All disturbed areas not stabilized with rock had been temporarily stabilized with seed & mulch. Entire access road and well pad had been rocked as outlined in the E&S plan. The operator should closely monitor the site and E&S controls to insure the site remains stable until well development activities are completed.
2101351 Primary Facility 2012-09-19 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well site was in the process of being set up for well stimulation. All access road and site erosion controls had functioned properly during the precipitation on 09/18/12 and there was no evidence of accelerated erosion or sedimentation observed in relation to the access road or well-site.
2103402 Primary Facility 2012-09-28 Complaint Inspection No Violations Noted Drilling had been completed and a service rig was located onsite monitoring flowback operations. The frac impoundment and two baker tanks were located onsite. All erosion controls appeared to have been maintained and were functioning properly. There was no observed evidence of accelerated erosion or sedimentation in relation to the access road or well-site. There was evidence observed of sediment transport and deposit on the vegetation in the neighboring yards both abovestream and downstream the well-site indicating that the sediment transport and deposit was part of the normal runoff flow for the area. According to an onsite conversation with MDS personnel the well-site is to be reclaimed within the next 7-10 days.
2108275 Primary Facility 2012-10-17 Routine/Complete Inspection No Violations Noted Drilling had been completed, above grade frac impoundment was in the process of being emptied. Site was being prepped for for final grading and plumbing the well for production. It was noted that th esilt fencing located onsite will be replaced with silt sox until final restoration is completed.
2248997 Primary Facility 2014-02-26 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been restored and the vegetative cover met the 70% uniform coverage criteria.
