2100952 Primary Facility 2012-09-19 Routine/Complete Inspection No Violations Noted At the time of inspection the well appeared to be drill and equipped for production. The associated well apparatus and brine tank located onsite appear to be in sound condition. No observation of accelerated erosion or sedimentation in realtion the well pad or access road. The E&S plan was onsite and BMPs were installed and functioning properly as outline in the E&S plan. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the site integrity is maintained.
2104932 Primary Facility 2012-10-03 Routine/Complete Inspection No Violations Noted Well operating and properly identified.
2108003 Primary Facility 2012-10-12 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well was not equipped for production. Frac impoundment located onsite and it had more than the required two feet of lined freeboard. There was no evidence observed of accelerated erosion or sedimentation related to the access road or well-site. It was noted that the sediment trap implemented at the outlets of the collection ditches required routine maintenance to insure its continued functionality.
2168122 Primary Facility 2013-05-15 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been preivously regraded and temporarily stabilized, however at the time of the inspection it was noted that the area that had made up the well-site was plowed for agricultural use.
2511158 Primary Facility 2016-08-04 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Brine tanks located onsite and they appeared to be in sound condition. It was noted that the well was located amidst a cultivated corn field.
