2104691 Primary Facility 2012-10-03 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not commenced at the time of the inspection. Access road and site E&S comtrols had been implemented as outlined in the onsite E&S plan. There were two issues with the controls as implemented. First the sediment trap installed at the end of the well-site collection ditch had disturbed area after the outflow of the trap. This would significantly reduce the effectiveness of the trap in minimizing the transport of sediment. Secondly the access road did not have enough culverts installed to effectively convey runoff off the access road. After a phone converstation with the operator and a field visit by the operator, the operator determined that a third culvert will be installed above the residence and measures will be implemented to insure that any runoff conveyed will not be directed toward the residence and the sediment trap will be extended so that it outlets to an undisturbed area.
2106293 Primary Facility 2012-10-10 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not commenced at the time of the inspection. Access road and site E&S comtrols had been implemented as outlined in the onsite E&S plan. The sediment trap implemented adjacent to the well pad had been extended out to the undisturbed area and an additional culvert had been implemented on the access road as discuessed via phone with a Snyder Brothers representative on 10/03/12. The E&S controls as implemented should effectively minimize accelerated erosion or sedimentation, provided the controls are properly maintained.
2139598 Primary Facility 2013-02-12 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not commenced at the time of the inspection. Access road and site E&S controls had been implemented as outlined in the onsite E&S plan. There was no observed evidence of accelerated erosion or sedimentation in relation to the access road or well-site.
2177840 Primary Facility 2013-06-25 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed. Drill pit located onsite and it contained fluids and drill cuttings. Drill pit had more than the required two feet of lined freeboard. There was no observed evidence of accelerated erosion or sedimentation in relation to the access road or well-site.
2205355 Primary Facility 2013-09-18 Site Restoration No Violations Noted Well appeared to be equipped for production. Brine tanks located onsite and they appeared to be in sound condition. Site had been properly restored and the vegetative cover met the 70% uniform coverage criteria. There was no observed accelerated erosion or sedimentation observed in relation to the site or access road.
2360271 Primary Facility 2015-04-10 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production, brine tanks located onsite and they appeared to be in sound condition. Site had been effectively restored and the vegetative cover met 70% uniform coverage. There was no observed accelerated erosion or sedimentation in relation to the site or access road.
