2204188 Primary Facility 2013-09-13 Routine/Complete Inspection No Violations Noted Site inspection performed as part of an OG-57 waiver application review. E&S plan as compiled should effectively minimize accelerated erosion and sedimentation, provided the controls are implemented and miantained properly. It was noted that drill cuttings are to be disposed of by encapsulation in the drill pit. Contact was made with the Operator by the Department and it was confirmed that during site construction a qualified professional will be onsite to document the soil conditions to insure the bottom of the drill pit is more than 20 inches above the seasonal high groundwater table to insure complaince with Chapter 78.56 and Chapter 78.62. The verification will then be forwarded to the Department as an amendment to the aproved E&S plan. Should the field verification show that the drill cuttings are not able to be encapsualted the operator will forward the appropriate change to the E&S plan detailing the chosen method of drill cutting disposal.

Perimeter E&S controls had been installed at the time of the inspection. There was no accelerated erosion or sedimentation in relation to the site or access road. The E&S plan as compiled should effectively minimize accelerated erosion or sedimentation provided the remaining erosion controls are implemented and maintained as outlined in the E&S plan.

2836044 Primary Facility 2019-02-04 Routine/Complete Inspection No Violations Noted On 2/4/19 at 1355 hrs., I conducted a routine inspection of MDS Energy Development, LLC's Ralph Myers etux 1M-35 well (API 005-31173) in Gilpin Twp., Armstrong Co. The well pad is restored and permanently stabilized. The well is equipped for production. A site ID sign is present at the entrance to the well pad. A well tag is affixed to the well head. There are no violations noted for this inspection.
