The site sign was present at the access road entrance right off SR 56 (Quaker Valley Road). I found old PPC plans, Emergency Response (ER) plan, and a well permit issued under previous owners (Chief and Chevron). I recommend that PPC and ER plans be updated to reflect current operator/contact information.

According to Department records, there is no ESCGP associated with the construction of the well pad and access road; only an E & S plan. The well pad remains at full built out size. Ditches and sediment traps have been installed per the E & S plans.

There is one drilled well onsite (009-20085). This well was issued regulatory inactive status (RIAS) on January 29, 2013. The regulatory inactive status just expired for well 009-20085. I recommend contacting the Departments Oil and Gas Inspector regarding extending this expired RIAS well or for other alternative methods.

2703423 Primary Facility 2018-03-09 Routine/Complete Inspection No Violations Noted The weather is 30 Deg. F, snowing and windy at time of inspection. Routine inspection of LPR Energy LLC., Davis Unit Pad, 1H Well on March 9, 2018.

There is one wells on this pad, currently shut in and has regulatory inactive status. Annulus is closed and vented into the poly tank at time of inspection. 100% LEL and 3% combustible gas was detected at the poly tank, 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. There is no production equipment in site. No violations noted at the time of inspection. DEP Representatives ¿ Ulyana Trynovich, Oil and Gas Inspector.

2841568 Primary Facility 2019-02-19 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection at LPR Energy's Davis Unit Pad on February 19, 2019. No one was onsite during my inspection.

According to Department records, there is no ESCGP associated with the construction of the well pad and access road; only an E & S plan. The well pad remains at full built out size. Ditches and sediment traps have been installed per the E&S plans. The entire site was snow/ice covered. No E&S issues observed.

There is one drilled well onsite (009-20085). This well was issued regulatory inactive status (RIAS) on January 29, 2013. An extension of RIAS was granted by the Department on May 14, 2018. The extension is only good for 1 year (May 13, 2019). No production equipment is onsite. There is currently no pipeline constructed to this well pad.
