There are 3 producing wells on this pad. Annulus is closed and not vented at time of inspection. Mechanical gauge on the production line reads pressure 500 PSI, the gauge on the annulus reads 0 PSI. 0% combustible gas was detected at the annulus or in the area immediately surrounding the well. The well pad has been partially reduced in size. The Department has approved the ESCGP NOT. Overall the well pad is stable and covered in snow.

Well site restoration reports were previously submitted by LPR in June 2016. The Department denied those reports because the well heads were not properly labeled per OAG3211(h). The Department recommending addressing the labeling issue and then resubmitting well site restoration reports for wells 013-20012, 013-20013, and 013-20015. These reports shall be submitted through the Departments Greenport system.
