Team Services Rig #126 is in the process of drilling out frac plugs on the 4H well. Fresh water, which is used in the process of drilling out frac plugs, had frozen in the well during drilling. The process which commenced today has been temporarily halted in order to heat the water according to Mr. Webb. There is a (330) gallon capacity tote container more than half empty of liquid stored on the ground surface near the access road. The tote is stored on top of soil excavated for an underground gathering pipeline. The liquid material in the tote may have been used to perform maintenance operations (pigging) of the pipeline according to Mr. Webb. A section of the containment berm in front of the Service Rig is not completely secured and some areas of compromised liner integrity were observed.

2607914 Primary Facility 2017-06-19 Routine/Complete Inspection No Violations Noted On June 19, 2017 at 1100 hrs., the Department conducted a routine inspection at the Chief Oil & Gas LLC, Stasiak Well Pad location in Pike and Stevens Townships, Bradford County. Andrew Hobbins, Water Quality Specialist, represented the Department. Nobody was present for Chief.

The site sign, E & S plan and weekly inspection logs were on site and available for review. The was observed to be well vegetated in certain areas and rock/shale lined in other locations. No erosion issues were observed.

Four wells were in production during the inspection. The wells were labeled and no bubbling was observed in the well cellars. Four production tanks and four GPUs were on secondary containment. Puddles on the pad surface tested low in specific conductivity. No staining was observed on the well pad surface.

On December 3, 2014 three violations were cited for puddles on the well pad surface that had elevated conductivity. Samples taken from the puddles confirmed the release of a pollutional substance. Fo

2637313 Primary Facility 2017-09-05 Site Restoration No Violations Noted On September 5, 2017, the Department conducted a site restoration inspection at the Chief Oil & Gas LLC, Stasiak Well Pad in Stevens Township, Bradford County. Andrew Hobbins represented the Department. Nobody was present for Chief. The inspection was in response to a Notice of Termination received by the Department on August 24, 2017.

Overall the site did not meet the requirements of 25 Pa Code 102.22(a)(2)(I) Permanent Stabilization. Two areas of the topsoil stockpile were not uniformly vegetated greater than 70%. In addition, the Department observed two compost filter sock BMPs installed at the eastern slope of the well pad. The Department recommends removing the E & S BMPs.

Based on the conditions observed during the inspection, it is recommended the NOT be denied.

2776767 Primary Facility 2018-09-18 Site Restoration No Violations Noted NOT approval. Chief vegetated the pad, now there is just a gravel road that loops around the wells.

I took electrical conductivity readings with an Oakton PCSTestr 35. Water on the pad is 0.30 mS/cm today. JLP
