2638069 Primary Facility 2017-09-14 Routine/Partial Inspection No Violations Noted At the time of inspection, I met with operator to discuss well. There is a pump jack on the well which is hooked up to electricity. The jack is in the down position, but operator stated that the well is pumped once a week. He keeps the power line on the ground because of constant tree falls. Well pumps to a tank which is uphill from the well. Integrity appears to be in good condition, with no signs of major corrosion causing any problems. Production hasn't been reported for well so instructions were sent via mail. Violation from prior inspection should be removed since proof of production was given.
2736146 Primary Facility 2018-05-30 Routine/Partial Inspection Violation(s) Noted On site with Scott Lux/DEP for a routine partial inspection.

While on site, Departmental staff observed brine and oil had been discharged at an unknown time from the production tank. This discharge is an unknown quantity and the discharge has happened and or reoccurred for an unknown amount of times. The discharge is flowing downhill into a wooded area resulting in dead vegetation and erosion. This discharge also threatens to pollute the Waters of the Commonwealth. Field tests with a soil probe conductivity meter show the soil conductivity in the discharge zone and path to be 19,000 microsiemens in the production diking/containment and 12,000 microsiemens in the discharge path downhill in the wooded area.

While on site it was also observed that the operator failed to construct and maintain adequate containment or diking for the production tanks and also failed to label well with the permit number.

2752163 Primary Facility 2018-07-02 Follow-up Inspection Outstanding Violations - Viols Req'd Met operator on site for a follow up inspection.

Discussed remediations and actions necessary for violation compliance. Operator had already planned to change out production tank pipping, rebuild tank diking, and remediate soils within diking. Department personnel recommended bio remediation for contaminated soils within diking and the areas contaminated by production fluid release. Operator expressed willingness for bio remediation and stated that he had access to compost, manure, and mulch for such purpose. Recommended bio remediation begins soon to give time before the first frost.

While on site, requested well tag be placed on well head.
