2712792 Primary Facility 2018-03-22 Routine/Partial Inspection No Violations Noted At the time of inspection, Dishong was onsite setting up a silt fence to control sediment from earth work. There is a stream just downhill of well and access road. Paperwork was going to be sent in to plug the well. He hopes to move the rig in tomorrow or Saturday, depending on the rain. A bit of work will be needed to allow access for bigger trucks to make it to well.
2719313 Primary Facility 2018-04-10 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, rig was set up at well and tubing was at bottom (2650'). Well was also logged. A small pit was dug and a liner was sitting next to it (to prevent rainwater from filling up pit and having more liquid to haul away). There were also 2 plastic totes and a 50 bbl poly tank. The entire access road now has a silt fence along it and gravel was added near the main road. Plan is to start cementing tomorrow.
2719315 Primary Facility 2018-04-11 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well has been cemented from TD (2650') up to 1300'. The liner for the pit was in the pit and had to be used. 30 bbls of water was circulated into well ahead of cementing and some oil came to surface. It was all captured in the pit.

Plugging operator will finish cementing once he returns from vacation.

2729237 Primary Facility 2018-05-09 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, well has been buried underground. 4 7/8" casing had been shot at 1250' and 50' was pulled out when a big cave occurred. Attempted to pull with 30,000 lbs, but no luck. Casing would not move back down the well either. It was decided to reshoot the casing at 1030'. All of that was pulled out. Casing remaining in well was from 1030'-1200'. Cement was pumped from 1300' up to 200' (was aiming for 300', but none of the cement was taken by formation). Pea gravel was poured from 200' to surface. API was welded on a steel plate and buried underground. Some reclamation needs done, but a bearing needs fixed on equipment. Silt fence will remain in place for the next year to allow ground to settle.
