No well completion report submitted and approved for this well. Well located in agricultural area. No well tag present at well head. Well assembly consists of 8 ¿pipe reduced to 4¿ reducer to 2¿ production line. Well head heavily corroded. Production equipment consisted of horizontal separator and tower. No pressure gauge present at well head. Two abandoned steel tanks remained at the site. Tanks were heavily corroded and have partially collapsed and were not connected for production. One 100 bbl poly tank present with multiple patches that showed leaking of brine water from the tank. Multiple patches showed leakage of brine water from patches. In addition, inspector detected brine staining of discharge to ground near poly tank. Access road to the well head shows considerable amount of erosion on the steep slope toward the well.
1847602 Primary Facility 2009-12-03 Compliance Evaluation Outstanding Violations - No Viols Req'd Well tag present at well head. Two abandoned steel tanks have been removed from site. Operator is in the process of removing current brine tank and replacing with new poly tank (was located at the adjacent Dominion¿s wells). The shared access road is in acceptable condition. However, the access road from shared road to the HA Farabaugh well is in poor condition and show considerable erosion and sedimentation problems. Inspector requested that the Water Quality personnel review the condition of access road and site. No E&S issues around the well head and production equipment. Discharge from current brine tank still exists.
