Well installed on 6/24/05 to depth of 8658¿ and WRAC report submitted and approved on 7/7/08. Well head consisted of 8¿pipe and bolted capped. At the time of inspection, one pit from drilling operations remained open at the site with limited plastic liner remaining along the top of the pit. Pit has filled with water. The cut face wall of the hill side pad has collapsed near the pit. Filter fence on the down gradient of the site pad is partially in place. E&S problems exist at the site and access road. Site has not been restored within the 9 month period from completion of well and well has been abandoned and was not placed in production within the 1 yr of completion of drilling.
1851677 Primary Facility 2009-12-22 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Met with Site Manager. Six trucks of fluid were removed from former pit. Site was in the process of restoration. Site pad¿s down gradient and up gradient locations were in process of regarding. Upon completion, site will be seeded and covered with straw. Well tag will need to be added to well. As per conversation with EQT, inactive status application was submitted to the PADEP for this well. Site work is to be completed by end of 2009.

1901064 Primary Facility 2010-07-26 Routine/Complete Inspection No Violations Noted Site relatively restored with min 70% vegetative growth, EXCEPT for a large area to the right of the well head (when standing at top of access road facing well). area in question may be where a frac pit was previously located and removed. The soil was soft, sunken and absence of vegetative growth (See attached pictures). I spoke John Centofanti of EQT via telephone on 7-27-10. Dept requested restoration report as well as method(s) of disposal for pit/contents.Dept is awaiting response from operator before proceeding. No additional accelerated erosion or sedimentation issues were noted in relation to well site (except for area noted above), although it is recommended that operator continue to maintain access road in order to prevent rutting. Noted that site was previously cited and assessed penalty for several violations on 12/03/09 Id#1847523. Conserv allowed the cement to setup and remaining 3028 to surface was going to be completed on 9/22/15.
2481392 Primary Facility 2016-04-21 Routine/Complete Inspection No Violations Noted At time of inspection well appears to be plugged.

Access road seeded with vegetation growing, but developing ruts. Department recommends Operator continue to monitor and maintain access road to avoid any E&S issues. Site seeded and mulched. No E&S issues observed on site. No E&S plan on site. Date of plugging completion not known therefore inspection is PENDING further investigation. UPDATE 11-16-16: Department OGI signed Certificate of well plugging on 4-16-16. Certificate of Well Plugging states plugging was completed on 9-24-15. Operator has 9 months from cessation of earth disturbance to permanently stabilize site. This inspection result is being updated to NOVIO. A follow up inspection will be conducted at a later date to ensure site is permanently stabilized.
