1804009 Primary Facility 2009-06-09 Routine/Complete Inspection Viol(s) Noted & Immediately Corrected Well is in the active-drilling phase. No signs of erosion and sedimentation on the access road and well site. A discharge of hydraulic fluid was discovered on the well site of approximately 5 gallons. The fluid spill was contained on site and addressed immediately. Pads were used to soak up the fluid on the surface. Then the contaminated soil was hand dug and placed in sealed drums. The operator was instructed to follow their PPC guidelines for the cleanup and disposal of the contaminated soil.
1805910 Primary Facility 2009-06-18 Compliance Evaluation No Violations Noted Well completed. Well head consisted of 7" pipe fitted to 4" pipe connected to shut off valve. No leakage of gas around well head. No signs of erosion and sedimentation on the access road and well site. A discharge of hydraulic fluid was removed from site pad. No sign of contaminated soil or material at the site. Pit aligned with plastic. No free board nor IW issues at the site. Frac tank, equipment trailer and sediment tank remain at the site. Drilling equipment removed from site.
1815548 Primary Facility 2009-07-24 Routine/Complete Inspection No Violations Noted Well is in post-drilling phase. There was erosion and sedimentation noted on culvert drain outlet. Outlet of culvert was not implimented properly, as a result sediment washed around the pile of stone placed near the outlet. See photos. The culvert outlet should have a rock lined sediment trap and outlet into an undisturbed area. Attention Vernon Fulmer.
